醒脑开窍法配合神经肌肉电刺激治疗脑卒中后吞咽障碍临床研究

Clinical Study on Method of Restoring Consciousness and Opening the Orifices Combined with Neuromuscular Electrical Stimulation for Dysphagia after Stroke

目的:观察醒脑开窍法与神经肌肉电刺激联合应用在卒中后吞咽障碍治疗中的临床价值。方法:选取79例脑卒中后吞咽障碍患者,按随机数字表法分为对照组(39例)和研究组(40例)。对照组给予醒脑开窍针刺治疗,研究组联合应用神经肌肉电刺激治疗。观察比较2组患者临床疗效、洼田饮水试验评分与吞咽功能量表评分改善情况。结果:研究组临床总有效率为92.50%,对照组为74.36%,2组比较,差异有统计学意义(P <0.05)。治疗前,2组洼田饮水试验评分比较,差异无统计学意义(P> 0.05)。治疗后,2组洼田饮水试验评分较治疗前降低(P <0.05);且研究组低于对照组(P <0.05)。治疗前,2组吞咽功能量表评分比较,差异无统计学意义(P> 0.05)。治疗后,2组吞咽功能量表评分较治疗前降低(P <0.05);且研究组低于对照组(P <0.05)。结论:醒脑开窍法联合神经肌肉电刺激治疗有助于改善吞咽障碍症状及吞咽功能,临床效果更佳。

Objective:To observe the clinical effect of the combined therapy of the method of restoring consciousness and opening the orifices as well as neuromuscular electrical stimulation in treating dysphagia after stroke.Methods:A total of 79 cases of patients with dysphagia after stroke were divided into the control group and the study group according to the random number table method,39 and 40 cases in each group respectively.The control group was given the acupuncture treatment for restoring consciousness and opening the orifices,and the study group was additionally treated with neuromuscular electrical stimulation based on the treatment of the control group.The clinical effect,the improvement of the scores of water swallow test and the scores of swallowing assessment in the two groups were observed and compared.Results:The total clinical effective rate was 92.50%in the study group and 74.36%in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of the scores of water swallow test in the two groups(P>0.05).After treatment,the scores of water swallow test in the two groups were decreased when compared with those before treatment(P<0.05);and the score in the study group was lower than that in the control group(P<0.05).Before treatment,there was no significant difference being found in the comparison of the scores of swallowing assessment in the two groups(P>0.05).After treatment,the scores of swallowing assessment in the two groups were decreased when compared with those before treatment(P<0.05);and the score in the study group was lower than that in the control group(P<0.05).Conclusion:The therapy of the method of restoring consciousness and opening the orifices combined with neuromuscular electrical stimulation can improve the symptoms of dysphagia and the swallowing function,which has better clinical effect.
