痛风伴高尿酸血症常见体质的中医护理指导效果与体会

Effect and Experience of Chinese Medicine Nursing Guidance on Patients with Gout Complicated with Hyperuricemia of Common Constitutions

目的:观察根据体质进行生活方式调护对痛风伴高尿酸血症疗效的影响。方法:将160例痛风合并高尿酸血症患者随机分为对照组和观察组各80例。2组患者均服用苯溴马隆片,并给予健康教育、生活方式和饮食结构调整等调护措施,观察组根据体质情况进行生活方式调护。2组疗程均为3个月,并进行3个月的随访。观察干预前后血尿酸(SUA)水平变化情况,并计算各时点的SUA达标率,评价患者对痛风知识的掌握程度,记录6个月内痛风急性发生情况。结果:干预后,2组SUA水平均较治疗前下降(P <0.01),观察组SUA水平低于对照组(P <0.01)。观察组SUA达标率42.50%,高于对照组的27.50%,差异有统计学意义(P <0.05)。观察组对痛风知识的掌握程度优于对照组(P <0.05)。观察组急性发作率46.25%,低于对照组的67.50%,差异有统计学意义(P <0.01)。观察组急性发作次数少于对照组(P <0.01)。结论:通过对痛风合并高尿酸血症患者进行中医体质学说指导下的生活方式调护,减少了痛风的急性发作,并且控制了SUA水平,提高了SUA的临床达标率,同时也提高了患者对痛风知识的知晓情况,有利于患者配合治疗。

Objective:To observe the effect of nursing guidance of lifestyle on gout complicated with hyperuricemia according to different constitutions of patients.Methods:A total of 160 patients with gout complicated with hyperuricemia were randomly divided into the control group and the observation group,80 cases in each group.Patients in both groups were given benbromarone tablets,and were intervened with health education,adjustment of lifestyle and diet and other nursing measures;the observation group was given nursing guidance of lifestyle according to different constitutions.Both groups were treated for three months and were followed up for three months.The changes of serum uric acid(SUA)before and after intervention were observed,and the compliance rate of SUA at each time point was calculated,the mastery of knowledge about gout in patients was evaluated,and the incidence of acute gout within six months was recorded.Results:After intervention,SUA levels in both groups were decreased when compared with those before treatment(P<0.01),and the one in the observation group was lower than that in the control group(P<0.01).The compliance rate of SUA was 42.50%in the observation group,higher than that of 27.50%in the control group,the difference being significant(P<0.01).Patients in the observation group had a better command of the knowledge of gout than those in the control group(P<0.05).The acute attack rate of gout in the observation group was 46.25%,lower than that of 67.50%in the control group,the difference being significant(P<0.01).The acute episodes of gout in the observation group was less than that in the control group(P<0.01).Conclusion:If patients with gout complicated with hyperuricemia receives nursing guidance of lifestyle according to different constitutions,the frequency of acute attack of gout can be reduced,SUA level can be controlled,and the clinical compliance rate of SUA can be improved.Meanwhile,the knowledge of gout of patients will be improved,which is conducive to their cooperation in treatment.
