中医内外合治促进手外伤术后患者康复临床研究

Clinical Study on Combined Internal and External Treatment in Chinese Medicine for Rehabilitation of Patients with Hand Injury after Operation

目的:观察中医内外合治对促进手外伤术后康复的临床效果。方法:将132例手外伤术后患者随机分为对照组和观察组各66例。2组患者均给予术后常规处理,对照组在常规处理的基础上进行功能训练,观察组则在对照组的基础上加用自拟生肌续筋汤内服、熏洗进行中医内外合治,连续用药1个月。对比2组患者临床治疗的优良率,对治疗前后上肢功能及徒手肌力进行评价。结果:观察组优良率高达83.3%,对照组71.2%,2组比较,差异有统计学意义(P <0.05)。治疗后,2组上肢功能上肢功能指数量表(UEF)I评分均较前改善(P <0.05)。观察组治疗后上肢功能UEFI评分高于对照组,2组比较,差异有统计学意义(P <0.05)。观察组徒手肌力较对照组改善明显,经秩和检验,差异有统计学意义(P <0.05)。结论:中医内外合治手外伤术后患者协同作用显著,能显著提高上肢功能,改善徒手肌力,凸显出中西医内外合治外伤术后患者的优势与特色。

Objective:To observe the clinical effect of combined internal and external treatment in Chinese medicine on promoting postoperative rehabilitation of hand injury.Methods:Divided 132 cases of patients with hand injury randomly into the control group and the observation group,66 cases in each group.The patients in both groups were given routine postoperative treatment;the control group was given functional training on the basis of routine treatment and the observation group was additionally given self-made Shengji Xujin tang and fumigation for combined internal and external treatment in Chinese medicine,with 1 month of treatment as one course.Compared the excellent and good rate of clinical treatment in the two groups and evaluated the upper limb function and the free hand muscle strength before and after treatment.Results:The excellent and good rate was 83.3%in the observation group and 71.2%in the control group,compared the two group,difference being significant(P<0.05).After treatment,the scores of upper limb function under the upper limb function index(UEFI)scale in the two groups were improved(P<0.05).The UEFI score of upper limb function in the observation group was higher than that in the control group after treatment,compared the two group,difference being significant(P<0.05).The free hand muscle strength in the observation group was significantly improved when compared with that in the control group,tested by rank sum test,difference being significant(P<0.05).Conclusion:The application of combined internal and external treatment in Chinese medicine has obvious synergistic effect in treating patients with hand injury after operation,which can significantly promote the function of upper limbs,improve free hand muscle strength,and highlight the advantages and characteristics of the combined internal and external treatment for patients with external injury after operation.
