腹针联合中药热奄包治疗脑卒中后便秘临床研究

Clinical Study on Abdominal Acupuncture Combined with Chinese Medicine Hot Pack for Patients with Constipation after Stroke

目的:探讨腹针联合中药热奄包对脑卒中后便秘患者胃肠道功能、排便情况及营养状况的影响。方法:将86例脑卒中后便秘患者按随机数字表法分为对照组和治疗组各43例,对照组给予中药热奄包外敷治疗,治疗组在对照组基础上采用腹针治疗。比较2组临床疗效、排便情况、胃肠道功能、营养状况。结果:治疗组总有效率为95.35%,明显高于对照组的79.07%(P <0.05)。治疗前,2组排便间隔时间、排便速度、排便性状、排便费力程度评分比较,差异无统计学意义(P> 0.05)。治疗后,2组排便间隔时间、排便速度、排便性状、排便费力程度评分均较治疗前明显下降(P <0.05),且治疗组各项评分均低于对照组(P <0.05)。治疗前,2组直肠静息压、肛门静息压、模拟排便动作肛门收缩压比较,差异无统计学意义(P> 0.05)。治疗后,2组直肠静息压、肛门静息压、模拟排便动作肛门收缩压较治疗前明显增高(P <0.05),且治疗组各项指标均高于对照组(P <0.05)。治疗前,2组血清白蛋白(Alb)、转铁蛋白(TRF)、前白蛋白(PA)测定值比较,差异无统计学意义(P> 0.05)。治疗后,2组Alb、TRF、PA测定值较治疗前明显升高(P <0.05),且治疗组各项指标均高于对照组(P <0.05)。结论:对脑卒中后便秘患者实施腹针联合中药热奄包治疗,临床疗效显著,可明显改善患者排便各指标及营养状况,有效促进胃肠道功能恢复,值得临床推广。

Objective:To discuss the effect of abdominal acupuncture combined with Chinese medicine hot pack on gastrointestinal function,defecation and nutrition in patients with constipation after stroke.Methods:A total of 86 cases of patients with constipation after stroke were divided into the control group and the treatment group according to the random number table method,43 cases in each group.The control group was treated with external application with Chinese medicine hot pack,and the treatment group was additionally treated with abdominal acupuncture based on the treatment of the control group.The clinical effect,defecation,gastrointestinal function and nutrition in the two groups were compared.Results:The total effective rate was 95.35%in the treatment group,significantly higher than that of 79.07%in the control group(P<0.05).Before treatment,compared the scores of intervals,speeds,characters and difficulty of defecation in the two groups,there was no significance in the difference(P>0.05).After treatment,the above scores in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the scores in the treatment group were lower than those in the control group(P<0.05).Before treatment,compared the resting pressure in rectum and anus,and systolic pressure in anus during defecatory maneuvers in the two groups,there was no significance in the difference(P>0.05).After treatment,the above three indexes in the two groups were significantly increased when compared with those before treatment(P<0.05),and the indexes in the treatment group were higher than those in the control group(P<0.05).Before treatment,compared the measured values of albumin(Alb),transferrin(TRF)and prealbumin(PA)in serum in the two groups,there was no significance in the difference(P>0.05).After treatment,the measured values in the two groups were significantly increased when compared with those before treatment(P<0.05),and the values in the treatment group were higher than those in the control group(P<0.05).Conclusion:The therapy of abdominal acupuncture combined with Chinese medicine hot pack in treating patients with constipation after stroke has significant clinical effect,which can significantly improve the indexes of defecation and nutrition and effectively promote the recovery of gastrointestinal function,and is worthy of clinical promotion.
