摘要
【目的】观察疏血通注射液联合鼠神经生长因子治疗糖尿病周围神经病变患者的临床疗效。【方法】将100例糖尿病周围神经病变患者随机分为研究组和对照组,每组各50例。2组患者均给予降血糖、控制血脂、饮食调控、运动管理等基础治疗,同时,对照组给予鼠神经生长因子治疗,研究组在对照组基础上给予疏血通注射液治疗。比较2组患者治疗前后正中神经和腓总神经的感觉与运动传导速度、多伦多临床评分系统(TCSS)评分、神经病变总体症状(TSS)评分以及血流动力学指标的变化情况,并评价2组疗效。【结果】(1)经2个疗程治疗后,研究组的总有效率为98.0%,对照组为84.0%,研究组的疗效明显优于对照组(P <0.05)。(2)治疗后,2组患者的正中神经和腓总神经的感觉与运动传导速度均较治疗前明显提高(P <0.01),且研究组对正中神经和腓总神经的感觉与运动传导速度的提高作用均明显优于对照组(P <0.01)。(3)治疗后,研究组的全血黏度、血浆比黏度和纤维蛋白原等血液流变学指标均较治疗前改善(P <0.05),而对照组患者各项指标较治疗前均无明显改善(P> 0.05),研究组对全血黏度、血浆比黏度和纤维蛋白原的改善作用均优于对照组(P <0.05)。(4)治疗后,2组患者的TCSS和TSS评分均较治疗前明显降低(P <0.05),且研究组的TCSS和TSS评分均明显低于对照组(P <0.05)。【结论】疏血通注射液联合鼠神经生长因子治疗糖尿病周围神经病变,可促进神经功能和临床症状的改善,提高临床疗效,而改善患者血液流变学水平可能是其起效的途径之一。
Objective To observe the clinical efficacy of Shuxuetong Injection combined with mouse nerve growthfactor(NGF) in treating diabetic peripheral neuropathy. Methods One hundred cases of diabetic peripheralneuropathy patients were randomized into study group and control group,50 cases in each group. Patients of thetwo groups were given fundamental treatment including decreasing blood glucose,controlling blood lipid,dietarycontrol and exercise management. Additionally, the control group was given intramuscular injection of mouseNGF, and the treatment group was given intramuscular injection of mouse NGF combined with intravenousdripping of Shuxuetong Injection. Before and after treatment, sensory nerve conduction velocity(SNCV)andmotor nerve conduction velocity(MNCV)of the median nerve and common peroneal nerve, Toronto clinicalscoring system(TCSS)scores, total symptom scores(TSS)for clinical neuropathy,and hemorheology indexesin the two groups were observed. And after treatment,the clinical efficacy of the two groups was evaluated. Results(1)After treatment for 2 courses,the total effective rate of the study group was 98.0%and that of the control groupwas 84.0%,the therapeutic efficacy of the study group being superior to that of the control group(P < 0.05).(2)After treatment, SNCV and MNCV of the median nerve and common peroneal nerve in the two groups wereremarkably increased as compared with those before treatment(P < 0.01),and the increase in the study group wassuperior to that in the control group(P < 0.01).(3)After treatment,hemorheology indexes such as the wholeblood viscosity, plasma viscosity and fibrinogen of the study group were obviously improved as compared withthose before treatment(P < 0.05), while hemorheology indexes of the control group showed no obviousimprovement(P > 0.05). The improvement of hemorheology indexes in the study group was superior to that in thecontrol group,the difference was statistically significant(P < 0.05).(4)After treatment,TCSS scores and TSS inthe two groups were decreased(P < 0.05 compared with those before treatment),and the decrease in the studygroup was superior to that in the control group(P < 0.05). Conclusion Shuxuetong Injection combined with mouseNGF is effective for the treatment of diabetic peripheral neuropathy by improving the nerve function and relievingthe clinical symptoms,and the improvement of hemorheology indexes may be one of its therapeutic mechanisms.
