洋地黄治疗心力衰竭伴心房颤动时出现长R-R间期的临床意义

Clinical significance of long R-R interval during digitalis treatment of chronic heart failure with atrial fibrillation

46例慢性心衰房颤患者,使用洋地黄治疗时心电图上R-R间期达1.5秒以上或见1秒以上的等长R-R。其中30例继续用维持量地高辛口服,28例并未发生洋地黄中毒的表现。提示上述心电图改变多数不是停用洋地黄的指征。

R-R interval of electrocardiogram were analysed in forty-six patients of chronic heart failure with atrial fibrillation during digitalis treatment. It has been shown that R-R interval during digitalis treatment is longer than 1.5 second, or same R-R interval is longer than one second. It was demonstrated that 30 eases continuously took low dose of digoxin(0.25mg per day). However, no digitalis poisoning occurred in 28 cases. The results suggest that electrocardiographic appearance described above is not a reliable sign of abstainning from digitalis in most patients.
