摘要
[目的]探索畸变产物耳声发射 (DPOAE)诊断职业性噪声性聋的可行性。[方法]选36例 (72耳 )噪声暴露青年工人作为实验组 ,11例 (22耳 )正常青年人作为对照组 ,进行纯音听阈和DPOAE幅值、信噪比和引出率测试。[结果]实验组纯音听阈测值在全部频率点均明显提高 ,与对照组比较差异有显著性 (P<0.05或P<0.01)。实验组DPOAE幅值和信噪比在全部频率点均明显下降 ,与对照组比较差异有显著性 (P<0.01)。实验组DPOAE总引出阳性率下降 ,与对照组比较差异有显著性 (P<0.01)。f0 为1000、6000、8000Hz时 ,DPOAE频点引出阳性率无明显下降 (P>0.05) ,而其他频点均明显下降 ,与对照组比较差异有显著性 (P<0.05或P<0.01)。[结论]DPOAE测试法诊断职业性噪声性聋是可行的 ,最佳的测试指标为DPOAE幅值。
To study the feasibility of professional noise_induced deafness diagnosed with distortion product otoacoustic emissions (DPOAE). Pure tone threshold and amplitude,SNR,positive rates of DPOAE were performed in 11 (22 ears) young adults with normal hearing of control group and in 36 (72 ears) young noise exposure workers of test group. Compared with the contorl group,in test group,pure tone threshold test values at all frequencies raised significantly (P<0.05 or P<0.01),test values of amplitude and SUR of DPOAE at all frequencies declined significantly (P<0.01), all positive rates declined significantly (P<0.01),and positive rates in 1 000,6 000,8 000 Hz of f0 declined no significantly (P>0.05),but in other frequencies f0 declined significantly (P<0.05 or P<0.01). [Conclusion] Professional noise_induced deafness can be diagnosed as DPOAE. The best test indicators are the amplitude,SNR and all positive rates of DPOAE.
