经阴道彩色多普勒超声在卵巢肿瘤鉴别诊断中的作用

The Role of Transvaginal Color Doppler Ultrasound in Differentiating Ovarian Tumor

目的 探讨经阴道彩色多普勒超声在卵巢肿瘤鉴别诊断中的作用。方法 对 78例卵巢肿瘤患者行经阴道彩色多普勒超声 (TVS -CD)检查 ,观察其血流显像情况 ,并与经腹部彩色多普勒超声 (TAS -CD)进行比较。结果 良性肿瘤血流显示率TVS -CD为 71.9% ,表现为 0～Ⅱ级 ,TAS -CD为 43 .8% ,0～Ⅰ级 ;恶性肿瘤血流显示率TVS -CD为 10 0 % ,TAS -CD为 97.8% ,均表现为Ⅱ～Ⅲ级 ,病理证实总符合率为 84%。结论 卵巢恶性肿瘤的血流显示率在TVS-CD及TAS -CD上均较高 ,且均表现为Ⅱ～Ⅲ级 ,但卵巢良性肿瘤的血流显示率TAS -CD明显低于TVS -CD 。

Objective To study the role of transvaginal color Doppler ultrasound (TVS-CD) in differentiating ovarian tumor. Methods 78 patients with ovarian tumor had undergone transvaginal and transabdominal color Doppler ultrasound (TAS-CD) to observe its blood flow in different conditions of these two examine methods. Results The blood flow showing rates of benign and malignant tumor were 71.9% and 100% on TVS-CD while 43.8% and 97.8% on TAS-CD. The degree of the blood flow of benign tumor was 0～2 grade on TVS-CD and 0～1 grade on TAS-CD while it were 2～3 grade both on TVS-CD and TAS-CD. The coincident rate with histology was 84%.Conclusion The showing rates of blood flow of ovarian malignant tumor are higher not only on TVS-CD but also on TAS-CD, but the showing rate and the degree of blood flow of benign tumor on TAS-CD are lower than on TVS-CD.
