膀胱移行细胞癌患者外周血UroplakinⅡmRNA检测的临床意义

The clinical significance of the detection of UPⅡmRNA in peripheral blood of patients with transitional cell carcinoma of bladder

目的 对膀胱移行细胞癌 (TCC)患者外周血中UPⅡmRNA进行检测并评价其临床意义。方法 应用逆转录聚合酶链反应 (RT PCR)检测外周血和组织中的UPⅡmRNA。结果 前列腺癌、肾癌、结肠癌、乳腺癌组织及 15例健康志愿者外周血中未见UPⅡmRNA的表达 ;正常膀胱移行上皮及癌组织中UPⅡmRNA表达率 10 0 % ;4 1例TCC肿瘤患者外周血中 14例表达阳性 ,阳性率为 34.1%。UPⅡmRNA的表达与临床分期关系为 :Tis 1期 10 % (1/ 10 )、T2 期 16 .7%(2 / 12 )、T3 期 5 0 % (4/ 8)、T4期 6 3.6 % (7/ 11) ;其中T3 、T4期与Tis 1、T2 期相比有显著性差异 (P <0 .0 1)。结论 UPⅡmRNA具有高度的组织特异性 ,可以作为诊断TCC血行转移的良好标志物 ,并能为临床治疗提供有利的实验依据。

Objective To evaluate the clinical significance of the detection of the peripheral blood in patients with transitional cell carcinoma of bladder.Methods UPⅡmRNA in peripheral blood and tissues was detected with Reverse Transcription-Polymerase chain reaction.Results Expression of UPⅡmRNA was negative in tissues of prostate cancer,renal carcinoma,colorectal cancer,breast cancer and peripheral blood of 15 healthy volunteers.Instead,in tissues of TCC and normal urothelium,the positive rate of the expression of UPⅡmRNA was 100%;UPⅡmRNA was also detected in peripheral blood of 14 out of 41 patients with TCC. The positive rate of the expression of UPⅡmRNA was 10%(1/10) in clinical stage T_ is-1 ,16.7%(2/12) in T_2,50%(4/8) in T_3,63.6%(7/11) in T_4. In clinical stage T_3 and T_4,the positive rate of the expression of UPⅡmRNA was significantly different with that in T_ is-1 and T_2.Conclusion UP is a highly specific histology marker to monitor metastasis of TCC.Detection of UPⅡmRNA can provide more helpful information for clinical to give patients with TCC better therapy.
