鼻腔无色素性恶性黑色素瘤的病理特征及鉴别诊断

Pathological features and differential diagnosis of amelanotic malignant melanoma in nasal cavity

目的 :探讨鼻腔原发性无色素性恶性黑色素瘤的病理特点及其鉴别诊断。方法 :对 9例鼻腔无色素恶性黑色素瘤进行光镜观察 ,并对全部病例作免疫组化染色。结果 :光镜形态以细胞形态的多形性和组织结构的多样性为主要特点 ,免疫组化染色 S- 10 0蛋白、HMB4 5及波形蛋白的阳性率分别为 10 0 %、88.9%和 77.8%。结论 :鼻腔无色素性恶性黑色素瘤组织学结构的变化特点有重要意义 ,免疫组织化学对其诊断及与鼻腔发生的其他肿瘤的鉴别诊断有重要价值。

Objective: To study the pathological characteristics of primary nasal cavity amelanotic malignant melanoma (PNCMMM) and its differential diagnosis. Methods: 9 cases of PNCMMM were investigated at the light microscopic level ,and all case were studied by immunohistochemical techniques. Results: The principal features were the variety of cellular patterns and histologic structures. Immunohistochemically, the positive rates of the expression of S-100 protein, HMB 45 and vimentin in 9 case of PNCMMM were100%, 88.9%and77.8%, respectively. Conclusions: The histo-pathological features of PNCMMM are of great significance,and immunohistochemical staining plays an important role in its diagnosis and differential diagnosis of other tumors of the nasal cavity.
