髋臼骨折的诊断与治疗

Diagnosis and treatment of acetabular fractures

目的 :探讨髋臼骨折的治疗方法。方法 :2 0例髋臼骨折采用重建钢板和钛螺钉内固定 ,其中后壁合并后柱骨折6例 ,前壁合并前柱骨折 3例 ,双柱骨折 7例 ,髋臼粉碎性骨折 4例。前壁合并前柱骨折采用髂腹股沟入路 3例 ,双柱骨折采用改良髂腹股沟入路 7例 ,余采用外侧扩大髂股手术入路。结果 :骨折均接近解剖复位 ,髋关节功能良好。结论 :手术前明确骨折类别 ,采用恰当的入路 ,术中充分的显露 ,满意的复位和可靠的内固定以及适度的功能锻炼是提高髋臼骨折治疗效果的关键。

Objective To investigate the surgical methods of acetabular fractures.Methods Twenty cases of acetabular fractures were treated with internal fixation using reconstruction plates and screws.Fractures of the posterior column and wall were found in 6 cases,anterior column and wall in 3 cases,both column in 7 cases,comminuted fractures in 4 cases.Ilio-inguinal approach was adoped in 10 cases,extended iliofemoral approach in 10 cases.Results Fracture obtained approximately anatomy reduction and joint obtained better function.Conclusion Correct classification of fracture,appropriate selection of operative approach,satisfactory reduction and strong internal fixation,appropriate function exercises are key points to improving outcome of acetabular fractures.
