严重烧伤患者血清胰岛素样生长因子-Ⅱ水平的变化及其与感染的关系

Change of the Serum IGFⅡ Level and its Relationship With Infection in Severe Burned Patients

目的 研究严重烧伤患者血清IGF -Ⅱ水平的变化规律 ,并探讨其与感染的关系。方法 采用放射免疫法检测烧伤患者血中IGF -Ⅱ水平。结果 与对照组比较 ,烧伤后第 5d患者血清IGF -Ⅱ水平呈显著性升高 (P <0 0 5～ 0 0 1) ,持续至伤后3 5d ;与伤后第 1d比较 ,伤后第 14、2 1和 2 8d血清IGF -Ⅱ水平呈显著性升高 ( P <0 0 5～ 0 0 1) ;与伤后第 3、5d比较 ,伤后第 14和2 1d血清IGF -Ⅱ水平呈显著性升高 (P <0 0 5 )。感染组IGF -Ⅱ水平显著高于非感染组 (P <0 0 1～ 0 0 5 )。患者血清IGF -Ⅱ水平与烧伤总面积和Ⅲ度烧伤面积无相关 (P >0 0 5 )。结论 IGF -Ⅱ参与了烧伤的发病机制 。

Objective To investigate the change of insulin-like growth factor Ⅱ(IGF-Ⅱ) and its relation with infection in severely burned patients.Methods RIA method was used to determine the serum IGF-Ⅱ level in patients with severe burn.Results Compared with control group, the level of IGF-Ⅱ was increased significantly at 5 postburn days , and persistent 35 days (P<0 05～0 01) .Comparison with the 1st day after burn, the levels of IGF-Ⅱincreased evidently at the 14th, 21st and 28th postburn days (P<0 05～0 01), and at 14 and 21 postburn days were obviously higher than those at 3rd and 5th postburn days(P<0 05). The IGF-Ⅱlevels of infection group were significantly higher than those of non-infection group (P<0 01～0 05).The level of IGF-Ⅱ was not correlated with total burn surface area (TBSA) and Ⅲ°BSA(P>0 05).Conclusion IGF-Ⅱ is involved in the pathogenesis of burn , and closely correlated with the infection after burn.
