腹腔镜子宫恶性肿瘤根治术64例分析

An Analysis of 64 Cases Undergoing Laparoscopic Radical Hys-terectomy and Pelvic Lymphadenectomy for Uterine Malignancies

[目的]总结腹腔镜下广泛性全子宫切除加盆腔淋巴结清扫术的实用价值。[方法]对35例子宫颈癌和29例子宫内膜癌行腹腔镜下广泛全子宫切除和盆腔淋巴结清扫术,分析其手术时间、术中出血量、淋巴结切除数目及术后恢复情况。[结果]平均手术时间289.92±72.6min,术中出血量370.0±315.0ml,切除淋巴结21.2±6.2枚,术后肛门排气时间34.19±9.13h,术后平均住院日12.9±4.8d。术中重要脏器损伤7例(大静脉损伤5例,膀胱损伤2例),尿潴留8例。[结论]腹腔镜下广泛性全子宫切除及盆腔淋巴结清扫术可作为早期子宫恶性肿瘤手术治疗的方法之一,但远期疗效有待进一步观察。

To evaluate the value of lapa ro scopic radical hysterectomy and pelvic lymphadenec-tomy.Thirty-five c ases with cervical carcinoma and29cases with endometrial carcinoma un-derwent l aparoscopic radical hysterectomy and pelvic lymphadenectomy.The operative time,i ntraoper-ative blood loss,postoperative recovery and number of lymph nodes exci sed were analyzed retrospec-tively.The mean operative time was289.92± 72.6min.The mean operative blood loss was370.0±315.0ml.Average number of pelvic lymph nodes excised was21.2±6.2.There were two cases of bladder injury,five ca ses of iliac vein injury,eight cases of urinary retention.Mean postoperative hos pitalization was12.9±4.8d.[Conclusion]Laparoscopic radical hysterectomy and pel vic lymphadenectomy may be one of the choices for the treatment of early stage u terine cancer,but the long-term effect should be fur-ther investigated.
