门静脉血栓的临床治疗分析

Treatment of portal vein thrombosis

目的 :探讨门静脉血栓的诊断及对不同时期门静脉血栓的治疗。方法 :回顾 16例门静脉血栓的不同治疗方法 ,4例行经皮经股动脉插管至肠系膜上动脉后 ,用尿激酶溶栓治疗 ;9例经外周静脉使用尿激酶溶栓治疗 ;2例行肠 -腔分流术 ;1例行食管胃底曲张静脉硬化剂治疗。结果 :14例症状明显缓解 ,1例症状无明显改善 ,1例出现肠坏死行坏死小肠切除。治疗过程中无死亡。结论 :该病的预后取决于早期诊断 ,及时治疗 ;对早期患者提倡介入溶栓治疗。

Objective:To explore the diagnosis and management of portal vein thrombosis in different stages.Methods:Sixteen cases of portal vein thrombosis were analysed restrospectively.Four cases received thrombolytic therapy with urokinase after intubation to superior mesenteric artery using seldinger technology was performed;9 cases were treated by thrombolytic therapy with urokinase via peripheral vein;2 cases underwent superior mesenteric vein caval shunt;1 case was treated with gastroesophageal varicose.Results:The symptoms were alleviated apparently in 14 cases; no improvement was noted in 1 case;and resection of part of the small intestine was performed in 1 case due to enteric necrosis.No death occurred during the treatment.Conclusions:The prognosis of portal vein thrombosis depends on early diagnosis and prompt treatment.The intervention thrombolytic therapy is advocated to patients with portal vein thrombosis of early stage.
