儿童腰椎骨折的内固定治疗

Internal fixation on fracture of lumbar vertebra in children

目的 通过对 7例儿童骨折脱位型腰段脊髓损伤的的诊断治疗分析 ,总结患儿受伤特点及内固定治疗的方法和经验。方法 回顾分析 7例急性腰椎骨折伴不完全脊髓损伤病例 ,男 5例 ,女 2例 ,年龄 6～ 1 2岁 ,平均 9岁。X线均呈明显腰椎骨折伴移位 ,按Frankel分级 ,B级 5例 ,C级 2例。采用后路减压内固定复位固定术。结果 经 8个月～ 4年 3个月随访 ,7例均恢复正常或接近正常 ,按Frankel分级提高 2～ 3级。结论 对于儿童不稳定腰椎损伤伴脊髓损害者非手术治疗无法复位并保持稳定应考虑及时采取手术治疗 ,采用精制内固定装置能有效减少创伤、重建脊柱的稳定性。

Objective To summarize the experience and treatment for fracture of lumbar vertebra with spinal injuries in children. Methods Seven cases of lumbar vertebra fracture complicat ed with partial spinal injury were analyzed retrospectively. There were 5 boys a nd 2 girls. Their age ranged from 6 to 12 years with an average of 9 years. Acco rding to Frankel classification, 5 cases were belonged to grade B and 2 to grade C. Results All patients underwent posterior decompression an d internal fixation, and were followed-up for 8 months to 4 years. All cases ha d complete recovery or improvement. Conclusion Decompression and internal fixation should be carried out in children with unstable lumbar vertebra fracture and spinal damage .
