摘要
目的 :应用超声心动图CK技术与用小剂量多巴酚丁胺结合 ,检测和评价心肌梗死区的心肌活性。方法 :将冠状动脉造影证实的34例心肌梗死患者于小剂量多巴酚丁胺用药前、用药中及冠脉血管成形术后3个阶段行CK技术检测。将梗死区的心肌活性 ,在时间和位移两方面的变化为依据的情况下进行识别和定量分析。结果 :心肌活性在用药前与后2个阶段比较 ,时间与位移的差别有统计学意义(P<0.01) ,检测心肌活性的敏感性96 43 % ,特异性66 67% ,准确性91 18 % ,阳性预测值93 10%。结论 :CK技术与小剂量多巴酚丁胺用药结合 ,能从时间和位移两方面做出定量分析 。
Objective:To detect and evaluate the viable myocardium from the myocardial infarction by using color Kinesis combined with low-dose dobutamine stress echocardiography(DSE).Methods:Thirty-four patients with myocardial infarction diagnosed by coronary angiography were detected for myocardial viability of post-myocardial infarction by quantifying CK images of the integrated display of the timing and magnitude of endocardial motion before pharmacological test,on trial and after coronary angioplasty.Results:There was a significant difference on the display of the timing and magnitude of endocardial motion between pre and post DSE(P<0.01).Sensitivity,specificity,accuracy,and positive predictive of CK images in distinguishing viability from myocardial infarction were96.43%,66.67%,91.18%,and93.10%respectively.Conclusion:The assessment of myocardial viability of post-myocardial infarction from quantifying the display of the timing and magnitude of endocardial motion can be completed by CK images and low-dose DSE.
