卵胞浆内单精子显微注射治疗男性不育的临床应用 被引量:5

摘要 目的 应用卵胞浆内单精子显微注射 (intracytoplasmicsperminjection ,ICSl)技术治疗因严重少 ,弱精及阻塞性无精症引起的男性不育。方法  74对不育夫妇经遗传咨询、染色体检查、Y染色体AZF基因分析、体检及生殖激素测定等 ,女方采用控制性超排卵技术获得卵子 ,男方少、弱精患者精液采用梯度离心法分离精子。阻塞性无精症患者采用经皮附睾穿刺或睾丸活检获得精子 ,选择成熟卵子行单精子显微注射。结果  82个治疗周期中共获卵 837个 ,其中成熟卵 712个 ,ICSI后 6 0 3个卵受精 ,受精率为 84 .6 9% ,发生卵裂 5 6 3个 ,卵裂率为 93.37% ,4 2w期获临床妊娠 ,临床妊娠率为 5 1.12 %。结论 ICSl技术是治疗男性不育的有效方法 ,患者接受治疗前应进行必要的遗传学检查和严格的遗传咨询。 Objective: to apply the technique of intracytoplasmic sperm injection (ICSI) to male infertility resulting from severe oligozoospermia asthenozoospermia and obstructive azoospermia. Methods: After genetic counseling, chromosome analysis, Y chromosome microdeletion analysis etc, 74 couples were accepted. Oocytes were collected from wives with controlled ovarian stimulation. Sperms from husbands with oligozoospermia or asthenozoospermia by density-gradient centrifugation, or from that with obstructive azoospermia by epididymal sperm aspiration or testicular sperm aspiration. Mature oocytes were fertilized by ICSI. Results: 837 oocytes were collected in 82 cycles. Among which 712 were mature. 603 were fertilized after ICSI. The fertilization rate was 84.69%. 563 oocytes developed to the stage of cleavage. The cleavage rate was 93.37%. Clinical pregnancy was achieved in 42 cycles. The pregnancy rate was 51.12%. Conclusions: The technique of ICSI is an efficient method for the male infertility therapy. Genetic examination and counseling are necessary for patients before ICSI.
