维拉帕米对麻醉犬的血流动力学影响

摘要 给11只麻醉犬恒速静脉灌注维拉帕米进行血流动力学测定。用药后,MAP、LVSP,HR 逐渐下降,PEP、RAP、LVEDP 逐渐增加,CO 呈先上升后降低的双相变化。在 CO 下降30%时,7只犬停药,2内上述各项指标有所恢复,4只继续用药犬全部死亡。说明维拉帕米对正常犬的心功能无明显抑制作用,但对心功能已受严重抑制者,可加剧血流动力学的紊乱,甚至造成死亡。 Verapamil was infused constantly to 11 dogs and their hemodynamics was measu- red.After the infusion,mean artery pressure(MAP),left ventricular systolic pressure (LVSP)and heart rate(HR)gradually decreased;pulmonary capillary wedge pressure (PWP),right atrial pressure(RAP)and left ventricular end-diastolic pressure (LVEDP)gradually increased;cardiac output(CO)presented a biphase change of initial elevation followed by a decline.By 30% decrement in CO,cardiac performance of 7 withdrawal dogs was partly improved and 4 dogs died.The results suggest that the intrinsic negative effect of verapamil in therapeutic dosage on cardiac function is not obvious in normal dogs,but it aggravates hemodynamic deterioration and even results in death in dogs with severely damaged cardiac function.
