胰岛素抵抗综合征甲襞微循环与球结膜微循环的临床研究

Clinical Study of the Insulin Resistance Syndrome Patients with Nail Fold Microcirculation and Bulbar Conjunctiva Microcirculation.

目的 :观察胰岛素抵抗综合征 (IRS)甲襞微循环(NFM )与球结膜微循环(BCM )的特点。方法 :IRS组 5 5例 ,健康对照组 3 0例 ,采用北京福凯公司生产的XTW—J微循环彩色显微分析仪 ,按田牛教授加权积分法计分设计 ,测定NFM与BCM的指标。结果 :IRS甲襞微循环与正常组比较 :管袢较少、输出支较宽、管袢较长 (P <0 .0 5 ) ;袢顶显著增宽、流速显著减慢 (P <0 .0 1) ;管袢交叉、管袢畸形、粗细不均、红细胞聚集、血色积分显著高于正常组(P <0 .0 1)。BCM与正常组比较 :囊状扩张与血色积分较高 (P <0 .0 5 ) ;缺血区、走行异常、网络结构、微血管瘤、微血流速、红细胞聚集、出血积分显著高于正常组(P <0 .0 1)。而NFM与BCM的形态积分、流态积分、袢周积分及总积分均显著高于正常组 (P <0 .0 1)。结论 :IRS甲襞微循环与球结膜微循环有中度至重度障碍。

Objective:To observe the features of Insulin Resistance Syndrome(IRS) Patients with Nail Fold Microcirculation(NFM) and Bulbar Conjunctiva Microcirculation(BCM).Methods:To 55 cases of IRS Patients Group and 30 cases of health control group, according to professor Tianniu's weighted integral ways, We measured indexes of NFM and BCM by using XTW-J microcirculation colour micro-analyses instrument made in Beijing Fukai Corporation.Results:Compared with health control group, features of IRS Patients,NFM are as follow: capillary loops less, export ramus is wider, capillary loops is longer(P<0.05). The top of capillary loops is obviously wider,the blood flow rate is obviously slower(P<0.01). The capillary loops cross, capillary loops misshape, different diameters of microvessels, the red blood cell aggregation and the score values of the blood color are obviously higher than health control group(P<0.01). For the features of IRS Patients,BCM are as follow: the score values of the capsular dilatation and the blood color are higher(P<0.05). The score values of the ischemic areas, the pathway abnormal, network structure, the micrangium neoplasm,the blood flow rate, the red blood cell aggregation and the blooding are obviously higher than health control group(P<0.01). The score values of the state, the flow and the blood vessel around and the integration are obviously higher than health control group(P<0.01).Conclusion:To IRS Patients,Nail Fold Microcirculation and Bulbar Conjunctiva Microcirculation have dysfunction from the middle to the bad.
