皮神经营养血管(丛)指背逆行岛状皮瓣的临床应用

Clinical application of dorsal digital reverse island flap with neurocutaneous vascular plexus

目的报道应用带皮神经营养血管(丛)的指背逆行岛状皮瓣修复指腹、指端侧方缺损及末节截指伤的临床效果。方法根据手指损伤的部位、形状、面积设计带皮神经营养血管(丛)指背逆行岛状皮瓣修复19例手指损伤创面。结果皮瓣全部成活,其中1例在术后36小时出现静脉危象,经皮缘拆线,伤口局部滴肝素钠等处理,4天后皮瓣颜色红润,肿胀消退。术后3～12个月随访皮瓣质地优良,外形与功能恢复满意。结论该手术方法操作简单、安全可靠,是修复手指创面的较理想皮瓣。

Objective To report clinical effect on repair defect of fingerpulp and fingertip by dorsal digital reverse is-land flap with neurocutaneous vascular plexus.Methods According to the site,shape and area of injury finger,we de-signed the dorsal digital island flap with neurocutaneous vascular plexus to repair surface of wound finger.Results All flaps survived,and1case happened venous crisis after36hours of operation,but through removing stitches and dripping heparin sodium,4days later the colour of flap returned red and had no swelling.Following-up for3-12months,the quality of flap were superior,their appearance and function were satisfactory.Conclusion With its simple procedure and reliable result,it is an ideal flap to re pair the surface of wound finger,because its safe and simple.
