严重烫伤后补体对外源性铜绿假单胞菌感染的影响

摘要 目的 探讨严重烫烧伤后血清补体趋化活性的动态变化及其对外源性铜绿假单胞菌感染的影响。方法 采用小鼠 30 %TBSAⅢ度烫伤模型 ,分为正常对照给菌组、单纯烫伤给菌组和去补体烫伤给菌组。检测血清补体趋化活性的动态变化 ,取肝、肺组织及心内血细菌培养 ,肝、肺组织进行细菌定量培养。结果 烫伤后血清补体的趋化活性在伤后 30min迅速升高 ,6h达到高峰 ;单纯烫伤给菌组肝、肺组织及心内血培养阳性率分别为 6 7%、4 8%、6 8% ,去补体烫伤给菌组分别为4 4 %、30 %、5 1% ;去补体烫伤给菌组肝、肺组织在伤后 12h的细菌量明显减少 ,与单纯烫伤给菌组比较相差显著 (P <0 0 5 )。结论 去补体后烫伤可减轻铜绿假单胞菌感染 ,提示过量的补体碎片可诱发外源性病菌感染。 Objective To study the dynamic changes of complement activation in postburn and their effects on exogenous P.aeruginosa infection in severe burned mice.Methods The mice inflicted 30%TBSA Ⅲ degree scalding were employed as model and randomly divided into normal control,scalding and decomplementizing scalding groups.The chemotactic activity of serum was tested after the mice were killed at 30 min,1,3,6,12,24 and 48 h in postburn,and the bacteria in liver, pulmonary organisms and heart blood bacterial were cultured and counted.Results The chemotactic activity of serum complements increased quickly within 30 min and reached the peak level at 6 hours in postburn.The P.aeruginosa in liver, lung and heart blood of scalding groups were 67%,48%,68% respectively and were 44%,30% and 51% in decomplementizing scalding group.In decomplementizing scalding group the quantity of bacteria in liver and lung decreased significantly( P <0 05)at 12 hrs in postburn.Conclusion The decomplementizing scalding could alleviate P.aeruginosa infection.Excess complement fragments of C3b and C5a could induce exogenous P.aeruginosa infection. [
