NBI联合醋酸染色在胃黏膜病变的诊断价值

摘要 目的:探讨内镜窄带成像(narrow-band imaging,NBI)联合醋酸染色对胃黏膜病变的诊断价值。方法:各种行胃镜检查的病例,对于粘膜不平、隆起糜烂、隆起凹陷病变者随机分成4组,分别用普通白光、普通白光+醋酸染色、NBI、NBI+醋酸染色,分别观察胃黏膜病变处腺管形态及微血管结构变化,进行病变清晰度评分,评价病变性质,并与病理检查结果相比较。结果::入选360例患者,NBI+醋酸染色观察病变时腺管结构及微血管走形变化更清晰,内镜与病理的诊断符合率高,显示NBI联合醋酸染色对病变微细结构的观察有明显优势。结论:NB I联合醋酸染色技术,达到优势互补,对病变初步定性及准确取材意义重大,提高活检检查的准确性,进而提高早期胃癌及癌前病变的检出率。
