七氟醚全麻复合硬膜外麻醉在肝脏手术中的应用

目的:探究肝脏手术患者应用七氟醚全麻复合硬膜外麻醉的效果,以便为临床工作提供指导。方法:选取2016年1月至2017年1月在我院接受肝脏手术的患者74例,按照麻醉方式的不同分成两组,对照组37例患者术中单纯给以七氟醚全麻,观察组37例患者术中给以七氟醚全麻复合硬膜外麻醉,观察两组患者的麻醉效果。结果:经麻醉诱导后,对照组T1-T4时刻的CVP(中心静脉压)明显高于观察组,经统计学分析,均存在统计学意义,P<0.05。术后第1天两组AST(谷草转氨酶)有显著升高,观察组术后第3天恢复正常,对照组术后第5天基本恢复;术后第1天、第3天两组患者的ALT(谷丙转氨酶)均显著升高,但相对而言,观察组较对照组幅度略低;术后第3天两组TBIL(总胆红素)均升高,且观察组较对照组偏低。数据经统计学软件分析后得出,P值均<0.05,有统计学意义。结论:肝脏手术患者应用七氟醚全麻复合硬膜外麻醉较单纯全麻来说效果更佳,值得临床推广应用。

Objective: To explore the effect of sevoflurane general anesthesia combined with epidural anesthesia in patients undergoing liver surgery in order to provide guidance for clinical work. Methods: Forty-seven patients were randomly divided into two groups according to the different methods of anesthesia. In the control group, 37 patients were treated with sevoflurane alone and 37 patients in the observation group Patients underwent epidural anesthesia with sevoflurane combined with epidural anesthesia to observe the anesthesia effect of the two groups of patients.Results: After induction of anesthesia, CVP(central venous pressure) was significantly higher in the control group than in the observation group at T1-T4, which was statistically significant(P <0.05). On the first day after operation, AST(aspartate aminotransferase) increased significantly, the observation group returned to normal on the third day after operation, the control group on the 5 th day after the basic recovery; postoperative day 1, day 3 ALT(alanine aminotransferase) were significantly increased, but relatively speaking, the observation group was slightly lower than the control group; the third day after the two groups TBIL(total bilirubin) were increased, and the observation group than the control group low. Data were analyzed by statistical software, P value were <0.05, statistically significant. Conclusion: The application of sevoflurane combined with epidural anesthesia in patients undergoing liver surgery is better than general anesthesia and is worthy of clinical application.
