医院-社区护理模式应用在分级诊疗中对糖尿病患者的影响

目的:探讨医院-社区护理模式应用在分级诊疗中对糖尿病患者的影响;方法:选取我院糖尿病患者90例,比较两组患者出院后3个月、6个月、1年的依从性和出院后1年患者血糖的控制水平;结果:患者出院后的依从性观察组显著优于对照组,出院后1年糖尿病控制情况明显优于对照组,P均<0.05;结论:医院-社区护理模式应用在分级诊疗过程中可有效控制糖尿病患者病情,提高患者依从性。

Objective:to explore the hospital-the model of community nursing in hierarchical diagnosis and treatment of diabetes patients; Methods:select our hospital 90 cases of diabetes patients, compared two groups of patients discharged from hospital after 3 months, 6 months, 1 year of compliance and 1 year after discharge control of blood sugar levels; As a result, patients were discharged after the adherence of observation group was significantly better than the control group, 1 year after discharge diabetes control situation was better than control group, P < 0.05; Conclusion:hospital-the model of community nursing in the process of grading diagnosis and treatment can effectively control the disease in diabetic patients, improve patient compliance.
