肾病综合征患者血清β_2微球蛋白水平与凝血、纤溶指标之间的相关性研究

Correlation between Serum Beta 2 Microglobulin Level and Coagulation and Fibrinolytic Parameters in Patients with Nephrotic Syndrome

目的探讨肾病综合征患者血清β_2-微球蛋白水平与凝血、纤溶指标之间的相关性。方法选择2016年2月至2018年2月我院收治的50例原发性肾病综合征患者作为试验组,选择同期健康受试者50例作为对照组,采集两组受试者的空腹静脉血,检测其β_2微球蛋白(β_2-MG)、凝血酶Ⅲ(AT-Ⅲ)、D-二聚体(DD)水平。比较试验组患者治疗前后上述指标的变化,并与对照组进行比较。结果试验组治疗前AT-Ⅲ水平明显低于治疗后及对照组,FIB、DD水平明显高于治疗后及对照组;试验组治疗前后FIB水平与β_2-MG水平呈正相关(P<0.05);AT-Ⅲ水平与β_2-MG水平呈负相关(P<0.05);DD水平与β_2-MG水平呈正相关(P<0.05)。结论血浆β_2-MG水平与AT-Ⅲ水平、FIB水平、DD水平密切相关,可以早期、及时、准确地反映肾病综合征患者的凝血及纤溶情况,评估患者的血栓风险,对监测患者的病情变化及治疗均具有指导意义,值得深入研究。

Objective This study aimed to investigate the correlation between serum beta 2-microglobulin level and coagulation and fibrinolytic parameters in patients with nephrotic syndrome.Methods A total of 50 cases of primary nephrotic syndrome in our hospital from February 2016 to February 2018 were selected as experimental group,and50 healthy subjects were selected as control group at the same time.The fasting venous blood of two groups of subjects was collected,and ALB,beta 2-MG,TC,DD,Cr,BUN,AT-Ⅲ,ATPP and PT were detected,and the indexes of the two groups were compared.The correlation of beta 2-MG.Results The indexes of ALB,beta 2-MG and AT-Ⅲin the test group were significantly lower than those of the control group.The TC,DD,Cr and BUN were significantly higher than those of the control group,and ATPP and PT were longer than those in the control group,and there were significant differences(P<0.01).Person correlation analysis showed that the level of beta 2-MG was strongly negatively correlated with the level of D-two polymer,APTT and PT in patients with nephrotic syndrome,and strongly positively correlated with the level of AT-Ⅲ.Conclusion Beta 2-microglobulin can early reflect renal function in patients with nephrotic syndrome,and it is related to coagulation and fibrinolysis in patients.Early assessment of patients’ bleeding and thrombus risk is of guiding significance.It is worthy of further study.
