腰椎动态关节松动术结合PNF技术对腰椎小关节紊乱的疗效观察

Clinical Observation of Mulligan’s Mobilization with Movement Combined With Proprioceptive Neuromuscular Facilitation in Patients with Lumbar Facet Joint Disorder

目的观察腰椎动态关节松动术(Mulligan)结合PNF(Proprioceptive Neuromuscular Facilitation)技术对腰椎小关节紊乱的疗效观察。方法将52名腰椎小关节紊乱的患者随机分为试验组和对照组各26例,两组都分别采用徒手腰椎动态关节松动技术(Mulligan),试验组同时增加PNF技术来增加躯干核心肌群与骨盆周围肌群稳定。两组均隔日治疗1次,共治疗3次。每次治疗前采用Oswestry腰痛残疾问卷ODI(The Oswestry Disability Index)及治疗前、后采用视觉模拟评分法VAS(Visual Analogue Scale)评定治疗效果。结果每位患者经过1周三次的治疗后,两组VAS评分及ODI值均较治疗前明显下降,差异有统计学意义(P<0. 05),且试验组干预前后的差值更大,差异有统计学意义((P<0. 05),患者的疼痛症状明显缓解,其日常活动能力提高。结论采用腰椎动态关节松动(Mulligan)结合PNF技术对腰椎小关节紊乱的疗效观察效果显著。明显优于徒手腰椎动态关节松动技术(Mulligan)。

Objective To observe the clinical efficacy of Mulligan’s mobilization with movement combined with proprioceptive neuromuscular facilitation(PNF)in patients with lumbar facet joint disorder.Methods Fifty-two patients with lumbar facet joint disorder were randomly assigned to experimental group(n=26)and control group(n=26).Mulligan’s mobilization with movement was applied in both groups every other day for a total of 3 times,while PNF was added only in experimental group to stabilize the core muscles and pelvic floor muscles.Clinical efficacies were assessed by the Oswestry Disability Index(ODI)before treatment and Visual Analogue Scale(VAS)before and after treatment.Results After treatment,ODI and VAS were significantly decreased in both groups(P<0.05)and these decreases were significantly higher in experimental group than control group(P<0.05).Significant improvement was also found in symptoms of pain and daily exercise capacity.Conclusion The combination of Mulligan’s mobilization with movement and PNF is effective in patients with lumbar facet joint disorder and better than Mulligan’s mobilization with movement alone.
