经皮撬拨复位空心钉内固定联合中药治疗青年难复位性股骨颈骨折

Irreducible Young Femoral Neck Fracture Treated by Close Nail-pry Reduction and Internal Fixation with Hollow Srews and Traditional Chinese Medicine

目的:探讨青年难复位性股骨颈骨折的复位方法。方法:将42例股骨颈骨折患者行经皮撬拨复位内固定及中药治疗,定期复查X片了解骨折愈合及股骨头坏死情况,并采用Harris评分系统进行髋关节功能评估。结果:42例均得到随访,平均临床愈合时间4.6月,弃拐时间3~8个月;术后所有病例切口均一期愈合;术后Harris评分(87.30±11.00)分,优38例,良2例,可2例。结论:经皮撬拨复位内固定治疗青年难复位性股骨颈骨折能在较小损伤的前提下达到要求骨位,骨折愈合率高,股骨头坏死发生率低,并发症发生率低,安全可靠,方法简单,值得推广。

Objective: To discuss the reset method of irreducible young femoral neck fracture. Methods: 42 cases of femoral neck fracture were treated by close nail-pry reduction and internal fixation with hollow screws and traditional Chinese medicine treatment,follow-up X-ray of the fracture healing and necrosis of the femoral head,and Harris score system was used to evaluate the function of hip joint. Results: 42 cases were followed up,the average clinical healing time of 4. 6 months,3 ~ 8 months post-operation time; all the cases healed after operation; postoperative Harris score( 87. 30 ± 11. 00) %,38 cases were excellent,good in 2 cases,fair in 2 cases. Conclusion: Close nail-pry reduction and internal fixation with hollow screws for the treatment of refractory young femoral neck fracture can reach the requirement of bone in the premise of small injury,high rate of fracture healing,low incidence of avascular necrosis of the femoral head. The low rate of complications,safe and reliable,simple method,is worthy of promotion.
