中医辨证护理对慢性阻塞性肺疾病临床症状、呼吸功能及生活质量的影响

Effect of Traditional Chinese Medicine Dialectical Nursing on Clinical Symptoms,Respiratory Function and Quality of Life for Patients with Chronic Obstructive Pulmonary Disease

目的:研究分析中医辨证护理对慢性阻塞性肺疾病(COPD)患者临床症状、呼吸功能及生活质量的影响。方法:对我院在2015年2月至2016年12月期间收治的94例确诊为COPD患者纳入研究对象,按照入院顺序将患者分为研究组(47例)和对照组(47例),对照组给予常规治疗及常规护理措施,研究组在对照组基础另给中医辨证护理治疗,观察两组治疗后临床症状及疗效,通过比较治疗前后BODE指数分值(预测COPD症状程度及预后情况指标)变化、肺功能指标[用力肺活量(FVC)、第一秒钟用力呼吸量(FEV_1)、最大自主通气量(MVV)]变化来判断其呼吸功能变化,并对两组治疗后生活质量变化(采用SGRQ评分)进行观察比较。结果:治疗后,研究组总有效率91.49%明显高于对照组59.57%(P<0.05),两组患者治疗后,BODE分值均较治疗前显著降低(P<0.05),且治疗后研究组BODE分值在治疗后1、3、6个月均较对照组降低明显(P<0.05),治疗后两组患者FVC、FEV_1、MVV及SGRQ生活质量评分均较治疗前显著升高,且治疗后研究组FVC、FEV_1、MVV及SGRQ生活质量评分较对照组均升高明显,具有统计学意义(P<0.05)。结论:中医辨证护理对慢性阻塞性肺疾病患者临床症状有较好的改善、对其呼吸功能及生活质量均有明显的提高,明显降低患者BODE指数分值,有效的提高患者肺功能及生活质量、临床疗效,值得临床借鉴和推广。

Objective: To investigate the effect of dialectical nursing on clinical symptoms,respiratory function and quality of life for patients with chronic obstructive pulmonary disease( COPD). Methods: A total of 94 patients with COPD were selected as subject admitted to our hospital from February 2015 to December 2016. The patients were divided into study group( 47 cases) and controlled group( 47 cases) according to the order of admission. The controlled group was given conventional treatment and routine nursing measures,while the study group was treated with TCM syndrome differentiation nursing based on the controlled group.Clinical symptoms and efficacy of the two groups were observed after treatment. The scores of BODE index before and after treatment( prediction of COPD symptoms and prognosis)( FEV_1) and maximum spontaneous ventilation( MVV) were used to determine the change of respiratory function,and the changes of quality of life in the two groups after treatment( using SGRQ Score)were observed and compared. Results: The total effective rate was 91. 49% in the study group was significantly higher than that in the controlled group( 59. 57%) after treatment( P<0. 05). The BODE scores were significantly lower than those before treatment in the two groups after treatment( P<0. 05). After treatment,the BODE score of the study group was significantly lower than that of the controlled group at 1,3 and 6 months after treatment( P<0. 05). The quality scores of life of SGRQ and FVC,FEV_1,MVV in both groups after treatment were significantly higher than those before treatment and the quality scores of life of SGRQ and FVC,FEV_1,MVV in the study group were significantly higher than those in the controlled group,the difference was statistically significant( P<0. 05). Conclusion: The dialectical nursing of traditional Chinese medicine has a better improvement in the clinical symptoms of patients with chronic obstructive pulmonary disease,and its respiratory function and quality of life are significantly improved,which can significantly reduce the BODE index score and effectively improve the patient's lung function and quality of life and clinical efficacy. It's worthy of clinical reference and promotion.
