中医护理对老年股骨颈骨折闭合复位内固定术后疼痛及关节恢复的影响

Effect of Traditional Chinese Medicine Nursing on Postoperative Pain and Joint Recovery after Closed Reduction and Internal Fixation for Elderly Femoral Neck Fractures

目的:观察老年股骨颈骨折闭合复位内固定术后疼痛及关节肿胀的护理过程中运用中医理论的护理效果。方法:探究对象选择2013年12月~2015年12月期间于我院骨科治疗的100例老年股骨颈骨折患者,均行闭合复位内固定术,随机均分为两组(观察组与对照组),对照组行常规护理,观察组在常规护理的基础上加用中医护理,比较两组疼痛(VAS)评分、髋关节Harris评分、住院时间、费用以及护理满意度。结果:观察组术后10d、20d VAS评分显著低于对照组,有统计学意义(P<0.05);术后30d差异不显著(P>0.05);观察组术后1月、3月Harris评分显著高于对照组,有统计学意义(P<0.05),术后6月无显著性差异(P>0.05);观察组护理满意度为98.00%,显著高于对照组(76.00%),有统计学意义(P<0.05);观察组住院时间、住院花费均显著低于对照组,有统计学意义(P<0.05)。结论:在老年股骨颈骨折闭合复位内固定术后疼痛及关节肿胀的护理中,采用中医辨证护理相对于常规护理在缓解疼痛,恢复关节功能方面具有明显的优势,同时能够减轻患者的经济压力,促进医患关系。

Objective: To study the nursing effect of the theory of traditional Chinese medicine on nursing process for postoperative pain and swelling of joint after closed reduction and internal fixation for elderly femoral neck fractures. Methods: 100 cases of patients with elderly femoral neck fracture treated in orthopedics of our hospital since December 2014 to December 2016 were selected as the study object and received closed reduction and internal fixation. These patients were randomly divided into two groups( observation group and controlled group),and patients in the controlled group were provided with routine nursing,while patents in the observation group were provided with,in addition to routine nursing,nursing of traditional Chinese medicine. The two groups were compared for pain( VAS) score,hip Harris score,length of stay,costs and nursing satisfaction. Results: The postoperative 10 d and 20 d VAS scores of the observation group were significantly lower than those of the controlled group,having statistical significance( P<0. 05). There was no significant difference between the two groups in 30 d after operation( P>0. 05)( The data collection was not real and not accurate due to the long period). The Harris sore of the observation group was significantly higher than that of the controlled group in 1 month and 3 months after operation,having statistical significance( P < 0. 05),and there was no significant difference in 6 months after operation( P > 0. 05). The nursing satisfaction of the observation group was 98. 00%,which was significantly higher than the controlled group( 76. 00%),having statistical significance( P < 0. 05). The length of stay and costs of the observation group were significantly lower than the controlled group,having statistical significance( P<0. 05). Conclusion: In nursing for postoperative pain and swelling of joint after closed reduction and internal fixation of elderly femoral neck fracture,nursing of traditional Chinese medicine has obvious advantage in relieving pain and recovering joint function compared with routine nursing,meanwhile,it can reduce economic pressure of patients and facilitate doctor-patients relationship.
