托里透脓汤与冰片加诺氟沙星联合治疗Ⅲ期、Ⅳ期压疮的临床效果及对其血清TNF-α、IL-6水平的影响

Clinical Effect of Toli Tounong Decoction Combined with Borneol and Norfloxacin for Treating Pressure Ulcers at Stage Ⅲ and Ⅳ and Influence on Serum TNF-α and IL-6 Levels

目的:观察托里透脓汤与冰片加诺氟沙星联合治疗Ⅲ期、Ⅳ期压疮的临床效果及对其血清TNF-α、IL-6水平的影响。方法:选取我院收治的Ⅲ期、Ⅳ期压疮患者104例作为研究对象,按照随机数字表法分为观察组和对照组各52例,2组均给予生理盐水清洗、碘液消毒等基础干预,在此基础上对照组给予冰片加诺氟沙星外敷治疗,观察组在对照组基础上联合托里透脓汤内服治疗。观察2组临床疗效及创面改善情况,评定疼痛程度,检测血清TNF-α、IL-6水平,并统计不良反应发生情况。结果:治疗后观察组临床总有效率显著高于对照组(P<0.05);观察组压疮总面积缩小率、总深度缩小率、总体积缩小率显著高于对照组(P<0.05);2组疼痛程度评分显著低于治疗前(P<0.01),且观察组显著低于对照组(P<0.01);2组血清TNF-α、IL-6水平较治疗前显著降低(P<0.01),且观察组显著低于对照组(P<0.01);2组治疗期间均未发生明显不良反应。结论:托里透脓汤与冰片加诺氟沙星联合治疗Ⅲ期、Ⅳ期压疮可有效改善患者临床症状和血清TNF-α、IL-6水平,提高治疗效果。

Objective: To observe the influence of Toli Tounong Decoction combined with borneol and norfloxacin for treating pressure ulcers at stage III and IV on clinical effect and serum TNF-α and IL-6 levels. Methods: 104 cases of patients with pressure ulcers at stage III and IV treated in our hospital were selected as the study objects,and were divided into observation group and controlled group according to random number table method,52 cases in each group. All the two groups were given physiological saline cleaning,iodine liquid disinfection and other basic intervention,on the basis of that,the controlled group were given borneol and norfloxacin external therapy,and the observation group were combined with Toli Tounong Decoction on the basis of the controlled group. Clinical efficacy and wound improvement were evaluated,pain degree was assessed,and serum TNF-α and IL-6 levels were examined,and adverse reactions were figured in the two groups. Results: After treatment,the total effective rate of the observation group was significantly higher than that of the controlled group(P<0. 05). The total reduction rate of the pressure sore,the total depth reduction rate and the volume reduction rate in the observation group were significantly higher than those in the controlled group(P<0. 05). Pain degree scores were significantly lower than those before treatment in the two groups(P<0. 01),and those in the observation group were significantly lower than those in the controlled group(P<0. 01). Serum TNF-αand IL-6 levels were significantly lower than those before treatment in the two groups(P < 0. 01),and those in the observation group were significantly lower than those in the controlled group(P< 0. 01). There was no obvious adverse reaction in treatment between the two groups. Conclusion: Toli Tounong Decoction combined with borneol and norfloxacin can effectively improve clinical symptoms and serum TNF-α and IL-6 levels in treatment of pressure ulcers at stage III and IV,and improve treatment effect.
