沈氏达郁汤加减方联合耳穴贴压治疗溃疡性结肠炎伴焦虑抑郁临床观察

Clinical Observation of Using Shenshi Dayu Decoction Addition and Subtraction Recipe Combined with Auricular Point Pressing in the Treatment of Ulcerative Colitis with Anxiety and Depression

目的:观察沈氏达郁汤加减方联合耳穴贴压治疗溃疡性结肠炎伴有焦虑、抑郁状态的临床疗效。方法:将84例溃疡性结肠炎伴焦虑、抑郁状态患者分为治疗组及对照组,每组42例,两组均接受美沙拉嗪肠溶片(1g,口服3/天)治疗。对照组给予口服艾司西酞普兰片10mg/次,1次/天,治疗组接受沈氏达郁汤加减方联合耳穴贴压治疗。治疗前后对每例患者中医症状、直肠黏膜评分、汉密尔顿焦虑、抑郁量表(HAMA、HAMD)评分,统计其临床疗效。结果:治疗后治疗组在中医症状总积分、直肠黏膜积分、HAMD总评分、HAMA总评分低于与对照组,比较有统计学意义(P<0.05)。治疗组、对照组临床疗效总有效率分别为92.86%、73.81%,治疗组临床疗效总有效率明显高于对照组,有统计学意义(P<0.05)。结论:沈氏达郁汤加减方联合耳穴贴压可以明显地改善溃疡性结肠炎患者中医症状及缓解焦虑、抑郁状态,明显提高其临床疗效。

Objective: To observe the effect of using Shenshi Dayu Decoction Addition and Subtraction Recipe combined with auricular point pressing therapy in the treatment of ulcerative colitis with anxiety and depression. Methods: 84 patients with ulcerative colitis with anxiety and depression were divided into treatment group and controlled group,42 cases in each group. The two groups received Mesalazin Enteric-coated Tablets(1 g oral 3/d) treatment. The controlled group was given orally escitalopram10 mg/times,1 times/d,the treatment group received Shenshi Dayu Decoction Addition and Subtraction Recipe combined with auricular plaster therapy. Before and after treatment,the TCM symptoms,rectal mucosa score,Hamilton Anxiety and Depression Scale(HAMA and HAMD) score of each patient was counted,and the clinical efficacy was evaluated. Results: After treatment,the total score of TCM symptoms,the integral score of rectal mucosa,the total score of HAMD and the total score of HAMA in the treatment group were lower than those in the controlled group(P< 0. 05). The total effective rate of the treatment group and the controlled group was 92. 86% and 73. 81% respectively,and the total effective rate of the treatment group was higher than that of the controlled group,with statistical significance(P<0. 05). Conclusion: Shenshi Dayu Decoction Addition and Subtraction Recipe combined with auricular plaster therapy can significantly improve symptoms in patients with ulcerative colitis and relieve anxiety and depression,improve the clinical curative effect.
