摘要 目的探讨尿白蛋白多聚体(PAs)在Ig A肾病发病机制中的作用。方法用免疫印迹法检测34例同步行肾活检诊断明确的Ig A肾病患者尿PAs,采用FM-8P全自动冰点渗透压计测量尿渗透压、邻苯三酚红法测量尿蛋白、肌氨酸氧化酶法测量血清肌酐(Scr)水平。结果 Ig A肾病尿PAs阳性患者肾小管间质损害评分、24 h尿蛋白定量及Scr水平均较阴性组升高(P均<0.05)。尿渗透压比较,尿PAs阳性Ig A肾病患者较尿PAs阴性者降低(P<0.05)。结论尿PAs对判断Ig A肾病肾小管间质损害程度有临床意义,可以作为判断肾功能恶化速度及预后的指标。 Objective To evaluate the role of urinary albumin polymers( PAs) in diagonosis and prognosis of IgA nephropathy( IgAN). Methods A total of 34 patients with biopsy proven Ig AN were studied. The existence of urinary PAs was detected by western blot. Data were processed with the classes of tubulointerstitial lesions and laboratory tests such as levels of osmotic pressure of urine,24 h urinary protein quantitation and serum creatinine( Scr). Results Scores of tubulointerstitial lesions,twenty-four-hour urine protein quantitation and Scr level were all significantly higher in urinary PAs positive group than in PAs negative group( all P < 0. 05); level of osmotic pressure was significantly lower in urinary PAs positive group than in PAs negative group( P < 0. 05). Conclusion Urinary PAs may have close relationship with the progression of IgAN,may be a useful index for the diagonosis and progonosis of IgAN.
