超声引导锁骨上臂丛神经阻滞麻醉效果观察 被引量:24

摘要 目的观察超声引导下经锁骨上入路臂丛神经阻滞的效果。方法选择90例拟行上肢手术的患者随机分为三组:传统方法组(T组)、神经刺激仪组(N组)、超声引导组(U组),每组30例。局麻药为1%盐酸罗哌卡因注射液和2%盐酸氯普鲁卡因注射液1∶1的混合液20 m L。观察三组桡神经、肌皮神经、正中神经、尺神经的阻滞起效时间和阻滞效果,评定手术全程的麻醉效果。结果 U组和N组各神经阻滞起效时间明显短于T组,且U组短于N组(P均<0.01)。U组和N组正中神经阻滞完善率明显高于T组(P<0.01),U组尺神经阻滞完善率高于N组和T组(P<0.01)。U组、N组麻醉优良率高于T组(P<0.05)。U组麻醉优等率(手术时完全无疼痛)明显高于N组、T组(P<0.05或<0.01)。三组并发症发生情况比较无统计学差异。结论超声引导锁骨上臂丛神经阻滞较神经刺激仪辅助及传统方法阻滞效果更好。
