梗阻性无精子症患者睾丸与附睾精子DNA完整性和顶体完整率、反应率对比分析 被引量:8

Comparisons of testicular and epididymal sperm DNA integrity rates,sperm acrosome integrity rate and reaction rate in patients with obstructive azoospermia

摘要 目的对比分析梗阻性无精子症(OA)患者睾丸与附睾精子DNA完整性和顶体完整率、反应率,为患者选取更好的精子助孕。方法 250例行卵胞质内单精子注射(ICSI)治疗的OA患者,经睾丸取精110例(A组)、附睾取精140例(B组),检测精子DNA完整性、顶体完整率及反应率。结果两组精子DNA完整性、顶体反应率比较,P均>0.05;A组精子顶体完整率为48.39%±22.46%,B组为40.10%±20.77%,P<0.05。结论睾丸与附睾的精子DNA完整性和顶体反应率无差异,但睾丸的精子顶体完整率优于附睾,行ICSI结局可能优于附睾精子。 Objective To compare and analyze the testicular and epididymal sperm DNA integrity rates,sperm acrosome integrity rate and reaction rate in patients with obstructive azoospermia( OA),and thus to choose better sperms for OA patients to the assisted reproduction. Methods A total of 250 cases of OA patients were treated by intracytoplasmic sperm injection( ICSI). The testicular sperm was obtained by TESA( group A,110 cases) and the epididymal sperm was obtained by PESA( group B,140 cases). We detected the sperm DNA integrity rate,sperm acrosome integrity rate and reaction rate. Results No significant difference was found in the sperm DNA integrity rate and sperm acrosome reaction rate between the two groups( all P > 0. 05). The sperm acrosome integrity rates of group A and group B were respectively 48.39% ± 22. 46% and 40. 10% ± 20. 77%,and significant difference was found between the two groups( P < 0. 05). Conclusions There are no significant differences in the testicular and epididymal sperm DNA integrity rates and sperm acrosome reaction rate,but the testicular sperm acrosome integrity rate is better than the epididymal one. The outcome of ICSI in the testicular sperm may be better than that in the epididymal sperm.
