摘要
目的分析肿瘤坏死因子α(TNF-α)基因多态性与脓毒症易感性及感染程度的关系。方法选择脓毒症患者80例,其中严重脓毒症32例(A组)、全身炎症反应综合征48例(B组),同期体检健康者50例(C组),采用聚合酶链反应—限制性片段多态性技术检测血清TNF-α基因在238、308位点的基因型。结果 A组TNF-α基因238G/A位点基因型分布频率GG 78.13%、GA 18.75%、AA 3.13%,等位基因分布频率G 87.50%、A 12.50%;B组分别为85.44%、14.58%、0、92.71%、7.29%,C组分别为88.00%、12.00%、0、94.00%、6.00%。A组TNF-α基因308G/A位点基因型分布频率GG 75.00%、GA 18.75%、AA 6.25%,等位基因分布频率G 84.38%、A 15.62%;B组分别为83.22%、14.58%、2.08%、90.63%、9.37%,C组分别为86.00%、14.00%、0、93.00%、7.00%。各组基因型、等位基因分布比较,P均>0.05。结论 TNF-α在238、308位点的基因多态性与脓毒症易感性及感染严重程度无明显相关性。
Objective To analyze the relationships of the gene polymorphism of tumor necrosis factor-α( TNF-α)with the susceptibility to sepsis and its infections degrees. Methods We selected 80 patients with sepsis,including 32 patients with severe sepsis( group A) and 48 patients with systemic inflammatory response syndrome( SIRS)( group B),and selected 50 healthy persons as the controls( group C). Polymerase chain reaction-restriction fragment length polymorphism( PCR-RFLP) was used to investigate the genetypes on site 308 and 238 of TNF-α. Results In the group A,the frequencies of GG,GA and AA genetypes on 238 G / A site of TNF-α were 78. 13%,18. 75% and 3. 13%; and the allele frequencies of G and A were 87. 50% and 12. 50%. In the group B,they were 85. 44%,14. 58%,0,92. 71% and 7. 29%; and88. 00%,12. 00%,0,94. 00% and 6. 00% in the group C. In the group A,the frequencies of GG,GA and AA genetypes on 308 G / A site of TNF-α were 75. 00%,18. 75% and 6. 25%; and the allele frequencies of G and A were 84. 38%and 15. 62%. In group B,they were 83. 22%,14. 58%,2. 08%,90. 63% and 9. 37%; and 86. 00%,14. 00%,0,93. 00% and 7. 00% in the group C. There were no statistically significant differences in the genetypes and allelic gene distribution among these three groups( all P > 0. 05). Conclusions The gene polymorphisms of TNF-α-238 and TNF-α-308 were not significantly related with the susceptibility and infectious degrees of sepsis.
