别嘌呤醇对高尿酸血症患者经皮冠状动脉介入治疗后对比剂肾病的预防作用 被引量:5

摘要 目的观察经皮冠状动脉介入治疗(PCI)前给予别嘌呤醇对高尿酸血症(HUA)患者对比剂肾病(CIN)的预防作用。方法将接受PCI治疗的155例HUA患者随机分为观察组75例和对照组80例,观察组术前24 h内口服别嘌呤醇300 mg。检测术前及术后1、2、3 d的血肌酐(Scr)、血尿酸(SUA)、肾小球滤过率(e GFR),留取术前、术后24 h尿液查尿酸排泄量,观察两组CIN发生情况。结果两组常规用药及对比剂用量比较,P均>0.05。观察组术后1、2、3 d的Scr、SUA低于对照组(P均<0.05),e GFR高于对照组(P<0.05)。两组术后24 h尿酸排泄量较术前增加(P均<0.05),但观察组增幅小于对照组(P<0.05)。观察组CIN发生4例(5.3%),对照组发生10例(12.5%),P<0.05。观察组未出现别嘌呤醇严重不良反应。结论别嘌呤醇可预防HUA患者PCI术后CIN的发生,可能其与降低SUA、改善肾功能有关。
