颈椎前路多椎间孔间隙显微减压术的生物力学评价 被引量:1

摘要 目的 :评价多椎间孔间隙单侧钩突切除术对颈椎稳定性影响的生物力学。方法 :6例成人新鲜颈椎标本 ,选择C4~ 7为实验节段。分正常组和手术组 ,在 2N·m纯力偶作用下分别产生前屈、后伸、左 /右侧弯和左 /右侧旋运动 ,通过脊柱三维运动测试来比较两种手术方式对颈椎稳定性的影响。结果 :①在 6个运动方向上 ,手术组术后ROM较术前无明显增加 (P >0 .0 5 ) ,差异无显著性 ;②运动范围的总和 ,手术组较术前有明显增加 ,差异具有显著性 (P <0 .0 1)。结论 :前路多椎间孔间隙显微减压术对颈椎静态稳定性的影响 ,主要表现在颈椎运动范围总和的增加 ,对某一方向ROM的影响不明显。 Objective: To evaluate the biomechanical effects of the unilateral cervical uncinate process resection on the stability of the cervical spine. Methods: Six adult fresh cervical specimens (C_ 4~7 ) were tested. The three dimensional motions of C_ 5~6 segments were analyzed to evaluate the immediate stability of cervical spine in both the normal state and the following resection state of unilateral uncinate process (RUUP). Results: In operation groups, the increase in the range of motion (ROM) was not significant in each direction, but the sum of the increasing ROM was significant in all cervical spines( P <0.01).Conclusions: Resection of three-levers unilateral uncinate process can increase the sum of ROM of cervical spine.
