摘要 目的 :探讨弥散加权成像 (DWI)和灌注加权成像 (PWI)在超急性和急性缺血性脑卒中的应用价值。方法 :复习和分析近期文献中有关超急性和急性缺血性脑卒中病例的DWI和PWI的影像表现。结果 :于超急性脑卒中 (6h以内 ) ,缺血性病变于DWI显示为高信号、于PWI显示为低灌注区 ,大多数病例可见到一半暗带。于急性脑卒中 (6~ 72h) ,随着时间的延长 ,此半暗带缩小或消失。结论 :临床同时应用DWI和PWI磁共振检查 ,有助于鉴别可恢复的或不可恢复的缺血性病变 ,从而有利于指导临床溶栓治疗。 Objective:To evaluate the clinical application of diffusion weight imaging (DWI)and perfusion weight imaging (PWI) in the super acute and acute ischemic stroke.Methods:Reviewing and analyzing the latest literatures about the DWI and PWI in patients with super acute and acute ischemic stroke.Results:The ischemic lesion in the super acute stroke(less than 6 hours), was showed as high signal on DWI and low perfusion area on PWI,a hemi opaque zone was found out in most patients.With the lasting acute stoke(6~72 hours),the hemi opaque zone was detected to decrease or disappear.Conclusion:Clinical applications of the MR DWI and PWI simultaneously are helpful to differentiating between the recoverable and non recoverable ischemic lesions,and guiding the clinical thrombolysis treatment.
