晕平方治疗肝阳上亢型慢性主观性头晕临床疗效评价

Clinical Efficacy Evaluation of Yunping Recipe in the Treatment of Chronic Subjective Dizziness with Syndrome of Hyperactivity of Liver Yang

目的:评价中药晕平方治疗肝阳上亢型慢性主观性头晕(CSD)的临床疗效。方法:采用随机对照方法,将符合纳入标准的CSD患者108例随机分为治疗组、对照组。两组均提供心理教育及盐酸舍曲林片(左洛复)治疗,治疗组另予口服中药晕平方,对照组口服乌灵胶囊,治疗期为12周。分别于治疗前及治疗后第4、8、12周进行眩晕残障程度评定量表(DHI)、综合医院焦虑抑郁量表(HAD)A+D项目、匹茨堡睡眠量表(PSQI)、疲劳严重度量表(FSS)及中医症状量表评分,并评价两组的中医证候临床疗效。结果:经重复测量资料的方差分析,两组不同治疗时间的DHI、HAD-A、HAD-D、PSQI及FSS积分均较治疗前显著降低(P<0.05);与对照组比较,治疗组第8、12周的DHI、HAD-A、HAD-D、FSS积分,第4、8、12周的PSQI积分均明显低于同期对照组(P<0.05)。在中医症状积分改善方面,治疗组在各观察点的积分均明显低于同期对照组(P<0.05),总体中医证候疗效优于对照组(P<0.05)。结论:晕平方可明显改善肝阳上亢型CSD患者临床症状,调节患者情绪,改善患者睡眠质量,减轻患者疲劳程度。

Objective: To evaluate the clinical efficacy of Yunping Recipe in the treatment of chronic subjective dizziness(CSD) with syndrome of hyperactivity of liver yang. Methods: By the randomized controlled trial,108 CSD patients who met the inclusion criteria were randomly divided into the treatment group and control group. Both groups were treated with psychological education and sertraline hydrochloride tablets(Zoloft). Additionally,the treatment group was orally treated with Yunping Recipe,and the control group was orally treated with Wuling capsule,with a course of 12 weeks. Before treatment and after treatment for 4,8 and 12 weeks,the scores of Dizziness Handicap Inventory(DHI),Hospital Anxiety and Depression scale(HAD) including anxiety related program(HAD-A) and depression related program(HAD-D),Pittsburgh Sleep Quality Index(PSQI),Fatigue Severity Scale(FSS) and TCM symptoms scale were evaluated,and the clinical efficacy of TCM syndrome in both groups was observed. Results: By the variance analysis of repeated measurement data,compared with the treatment before,the scores of DHI,HAD-A,HAD-D,PSQI and FSS were all significantly decreased in both groups after different treatment time(P < 0. 05). After treatment for 8 and 12 weeks,the scores of DHI,HAD-A,HAD-D and FSS in the treatment group were obviously lower than those in the control group,and after treatment for 4,8 and 12 weeks,the PSQI scores in the treatment group were obviously lower than those in the control group(P < 0. 05). The scores of TCM symptoms in the treatment group at each observation point were obviously lower than those in the control group(P < 0. 05). The total efficacy of TCM syndrome in the treatment group was better than that in the control group(P < 0. 05). Conclusion: Yunping Recipe can significantly improve the clinical symptoms of CSD patients with syndrome of hyperactivity of liver yang,regulate the mood,improve the quality of sleep and alleviate fatigue of patients.
