自拟干眼熏洗方治疗睑板腺功能障碍的疗效观察

Effects of “Ganyan Fumigation Decoction” in the treatment of meibomian gland dysfunction

目的:观察干眼熏洗方治疗睑板腺功能障碍(MGD)的疗效。方法:选取56例(112眼)MGD患者,随机分成治疗组30例(60眼)和对照组26例(52眼)。两组患者均给予常规眼部清洁、热敷、睑板腺按摩,以及典必殊眼膏外涂,0.1%玻璃酸钠滴眼液滴眼;治疗组在此基础上加用干眼熏洗方熏洗治疗。两组疗程均为1个月。比较两组患者的主要临床症状积分、眼表面疾病指数(OSDI)评分、角膜荧光染色(CFS)评分及睑板腺分泌能力和分泌物质量评分,检测两组患者的泪膜破裂时间(BUT)、泪液分泌试验(SIT)。结果:治疗后,两组患者的主要临床症状积分、OSDI评分、CFS评分及睑板腺分泌能力和分泌物质量评分较治疗前均显著降低(P<0.01),且治疗组患者的评分均明显低于对照组(P<0.01)。治疗后,两组患者的BUT和SIT检查结果均显著改善(P<0.01),且治疗组患者的改善程度优于对照组(P<0.01)。结论:干眼熏洗方能明显缓解MGD引起的眼部不适症状,改善睑板腺分泌功能,是一种可行的治疗方法。

Objective: To observe the therapeutic effects of "Ganyan Fumigation Decoction"in the treatment of meibomian gland dysfunction( MGD). Methods: 56 patients( 112 eyes) with MGD were selected and randomly divided into the treatment group( 30 cases,60 eyes) and the control group( 26 cases,52 eyes). All the patients were treated with daily cleaning,warm compress and massage,as well as tobradex eye ointment and 0.1% sodium hyaluronate eye drops for external use. The patients in the treatment group were treated with fumigation of"Ganyan Fumigation Decoction"based on above. The treatment course was 1 month. The scores of main symptoms,ocular surface disease index( OSDI),corneal fluorescein staining( CFS) test,and meibomian gland secretory capacity and secretion quality were compared,and fluorescein tear break-up time( BUT) and Schirmer I test( SIT) were detected. Results: After treatment,the scores of main symptoms,OSDI,CFS and meibomian gland secretory capacity and secretion quality in both groups were obviously decreased( P < 0.01),and the scores of the treatment group were lower than those of the control group( P < 0.01). After treatment,the results of BUT and SIT in both groups were significantly improved( P<0.01),and the improvement of the treatment group was superior to the control group( P < 0.01). Conclusion: "Ganyan Fumigation Decoction"can obviously relieve the ocular symptoms caused by MGD and improve the meibomian gland secretory function,which is an effective and feasible therapy.
