何庆勇运用甘麦大枣汤的经验

He Qingyong's Experiences in the Application of Ganmai Dazao Tang

经方历史悠久,用之得当,效如桴鼓。文章较全面叙述了何庆勇副教授运用经方甘麦大枣汤治疗疑难杂症的临床经验。甘麦大枣汤出自《金匮要略·妇人杂病脉证并治》:"妇人脏躁,喜悲伤欲哭,象如神灵所作,数欠伸,甘麦大枣汤主之。"此方是医圣仲景为脏躁而作,条文主要是论述脏躁(更年期),情志郁结,血虚阴亏,心神失养,内火扰神而现喜悲伤欲哭。总结历代医家对此条文的理解,结合何庆勇老师多年临床经验,笔者指出运用甘麦大枣汤取佳效的关键在于:一是辨方证,凡是符合"喜悲伤欲哭"方证者,不拘妇人、男士或是儿童,均可用之;二是坚持原方原量,原方中的甘草、浮小麦、大枣三药不可缺一,浮小麦必须用仲景本源剂量;三是应注重经方叠用。

The classic formulas have a long practical history and the effect is perfect when used properly. Prof. He Qingyong's clinical experiences were introduced in the paper for the treatment of incurable diseases with ganmai dazao tang. Ganmai dazao tang is from Jinkui yaolue: furen zabing maizheng bingzhi. It is mentioned in the book that ganmai dazao tang is applicable for female zangzao disorder and mental disorder. Zhang Zhongjing who is named as the medical sage invented this formula for zangzao disorder. Mental disorder happens in zangzao disorder( menopause stage),induced by liver qi stagnation,blood and yin deficiency,malnutrition of heart and disturbance of internal fire. By summarizing the experiences of medical scholars in different dynasties and in association with Prof. He's experiences,the writers pointed out that the excellent effect of ganmai dazao tang was achieved because of accurate syndrome differentiation of mental disorders for males,females or children,keeping on the original application of formula in composition and dose,such as glycyrrhiza,fructus tritici levis and fructus ziziphi jujubae,and Zhang Zhongjing' s original dose of fructus tritici levis; and emphasizing the overlapping of classic formulas.
