中西药联合修复癫痫的异常灌注灶临床研究

Clinical Research of Abnormal Perfusion Foci of Epilepsy Repaired with Integrated Chinese and Western Medicine

目的探讨中药联合抗癫痫药(AEDs)修复癫痫患者的异常灌注灶的必要性和有效性。方法中药联合AEDs组1016例,进行治疗前后的发作间期单光子发射电子计算机断层扫描(SPECT)显像、长程视频脑电图监测(V-EEG)与影像学检查。结果中药联合AEDs组临床发作控制时间平均30.9个月。SPECT显像阳性880例(86.6%),修复后异常663例(65.2%),正常增加353(34.7%),异灶减少506(40.9%)。中药联合AEDs平均修复率为34.7%,AEDs组为11.7%(P<0.05),特发组(66.44%)与症状组(27.52%)比较差异有统计学意义(P<0.01)。结论中药联合AEDs修复癫痫的异常灌注灶是有效的,发现癫痫性质与有修复特性的中药是成败的关键。

Objective To explore the necessity and effectiveness on abnormal perfusion foci in epilepsy patients treated with Chinese medicine and anti- epilepsy drugs( AEDs). Methods One thousand and sixteen cases in Chinese medicine and AEDs group received SPECT imaging in the seizure before and after treatment,long- term EEG and imaging tests. Results The average seizure control time was 30. 9 months in Chinese medicine and AEDs group. Of the cases,880 cases( 86. 6%) presented SPECT imaging positive rate;after repair,663 cases( 65. 2%) presented abnormal perfusion foci,the normal case numbers were increased up to 353 cases( 34. 7%) and the abnormal foci were reduced in 506 cases( 40. 9%). The average repair rate was 34. 7% in Chinese medicine and AEDs group,was 11. 7% in AEDs group( P < 0. 05) and was 66. 44%in the idiopathic group,presenting the significant difference as compared with the symptom group( 27. 52%,P< 0. 01). Conclusion Chinese medicine combined with AEDs achieves the effective results in repairing epileptic abnormal perfusion foci. It is discovered that the seizure property and Chinese medicine of repair characteristics are the keys for the successful treatment.
