丹参多酚酸盐对高龄老年急性脑梗死患者凝血功能的影响

Impact of Salvianolate on Coagulation Function in Senile Acute Cerebral Infarction

目的探讨丹参多酚酸盐对高龄老年急性脑梗死患者凝血功能的影响。方法将183例80岁以上急性脑梗死患者随机分为两组,对照组93例予以西医常规治疗,治疗组90例在常规治疗基础上加用丹参多酚酸盐200 mg入0.9%氯化钠注射液250 ml或5%葡萄糖注射液250 ml静脉滴注,日1次,治疗14 d。观察两组患者治疗前后凝血指标[血小板计数(PLT)、D-二聚体(D-D)、凝血酶原时间(PT)、活化的部分凝血活酶时间(APTT)、纤维蛋白原含量(FIB)、凝血活酶时间(TT)]的变化情况。结果两组患者治疗后FIB和D-D均较治疗前显著回落,差异有统计学意义(P<0.05);而且治疗组治疗后FIB和D-D均较对照组治疗后下降更为明显,差异有统计学意义(P<0.05)。两组患者APTT、PT、TT在治疗后均较治疗前有所延长,PLT治疗后均较治疗前有所增加,但两组治疗前后以及组间比较差异无统计学意义(P>0.05)。结论丹参多酚酸盐能够有效降低高龄老年急性脑梗死患者血FIB和D-D浓度,有助于缓解高凝状态。

Objective To discuss the impact of salvianolate on coagulation function in senile acute cerebral infarction. Methods One hundred and eighty - three patients of acute cerebral infarction,over 80 years old,were randomized into two groups. In the control group(93 cases),the routine western medicine was applied. In the treatment group,besides the basic routine treatment,the intravenous drip was applied with salvianolate 200 mg and 0. 9% sodium chloride injection 250 ml or 5% glucose injection 250 ml,once a day for 14 days. The coagulation indicators(PLT,PT,APTT,FIB and TT)were observed before and after treatment in the two groups. Results FIB and D - D after treatment were reduced significantly as compared with those before treatment in the two groups,indicating the significant difference(P < 0. 05). In the treatment group, FIB and D - D were reduced much more apparently as compared with the control group after treatment,indi-cating significant difference(P < 0. 05). APTT,PT and TT were prolonged after treatment as compared with those before treatment in the two groups. After treatment,PLT was increased as compared with that before treatment. But,the differences were not significant before and after treatment of the two groups and between the two groups(P > 0. 05). Conclusion Salvianolate effectively reduces FIB and D - D concentration and benefits in relieving hypercoagulability in senile acute cerebral infarction.
