急性冠脉综合征血瘀证与RAAS系统及微循环的关系研究

Study on the relationship between the blood stasis syndrome of acute coronary syndrome and RAAS system and microcirculation

目的研究及分析急性冠脉综合征血瘀证与RAAS系统及微循环的关系,为该病的治疗提供监测依据。方法选取2013年1月-2015年11月接受治疗的52例急性冠脉综合征血瘀证患者为A组,52例急性冠脉综合征非血瘀证患者为B组,52名同龄健康人员为C组,然后将3组研究对象的血清RAAS指标及甲襞微循环指标进行比较,并比较A组中轻度、中度及重度血瘀证患者的上述指标,同时采用Logistic分析血清RAAS指标及甲襞微循环指标与急性冠脉综合征血瘀证的关系。结果 A组血清RAAS指标中RA、Ang-Ⅰ、Ang-Ⅱ、ALD及AT2R水平均高于B组及C组,B组血清RA、Ang-Ⅰ、Ang-Ⅱ、ALD及AT2R水平则高于C组,且A组中轻度、中度及重度血瘀证患者血清RA、Ang-Ⅰ、Ang-Ⅱ、ALD及AT2R水平比较差异有统计学意义(P<0.05)。A组的输入枝长度及管径、输出支长度及管径均低于B组及C组,且B组低于C组,而白微栓率A组高于B组及C组,且B组高于C组。且A组中轻度、中度及重度输入枝长度及管径、输出支长度、管径及白微栓率比较差异有统计学意义(P<0.05)。经Logistic分析显示血清RAAS指标及甲襞微循环指标均与急性冠脉综合征血瘀证有密切的关系。结论急性冠脉综合征血瘀证患者的RAAS系统及微循环呈现相对较差的状态,上述指标均与本病有密切的关系,应加强对上述指标的监测。

Objective To study and analyze the relationship between the blood stasis syndrome of acute coronary syndrome and RAAS system and microcirculation,in order to provide monitoring basis for the treatment of this disease. Methods 52 cases of patients with blood stasis syndrome of acute coronary syndrome in our hospital from January 2013 to November 2015 were selected as group A,and 52 cases of patients with acute coronary syndrome and without blood stasis syndrome were selected as group B,52 healthy persons were selected as group C,then the serum RAAS indexes and nail fold microcirculation indexes of three groups were compared,then those indexes of group A with mild,moderate and severe blood stasis syndrome were compared too,then analyze the relationship between serum RAAS indexes,nail fold microcirculation indexes and blood stasis syndrome of acute coronary syndrome by the Logistic. Results The serum RAAS indexes of group A were all worse than those of group B and group C,the indexes of group B were all worse than those of group C,the nail fold microcirculation indexes of group A were all higher than those of group B and group C,the indexes of group B were higher than those of group C,and those indexes of group A with mild,moderate and severe blood stasis syndrome all had obvious differences too,and the serum RAAS indexes and nail fold microcirculation indexes all had close relationship to the blood stasis syndrome of acute coronary syndrome.Conclusion The RAAS system and microcirculation of patients with blood stasis syndrome of acute coronarysyndrome show relatively worse state,and those indexes all have close relationship to the disease,so those indexes should be paid more monitoring.
