电针夹脊穴配合调制中频治疗老年带状疱疹后遗神经痛疗效观察

Clinical Efficacy Observation of Senile Post- Herpetic Neuralgia Treated with Electroacupuncture at Jiaji( EX- B2 ) and Modulated Medium Frequency Stimulating Therapy

目的探讨电针夹脊穴配合调制中频电治疗老年带状疱疹后遗神经痛(PHN)的临床效果。方法按照随机数字表法将PHN患者随机分为两组,每组38例,治疗组采用电针夹脊穴配合调制中频电治疗,对照组采用常规局部围刺配合电针治疗,于治疗前、治疗5次、10次、15次及20次后测定比较两组患者的视觉模拟评分(VAS)、汉密尔顿焦虑量表(HAMA)评分及临床疗效。结果治疗2个疗程后,治疗组的显愈率明显高于对照组,差异有统计学意义(P<0.05);治疗组患者在不同时段的VAS评分明显低于对照组同时段水平及自身治疗前水平;而对照组在治疗10次以后VAS评分也明显低于自身治疗前水平,差异均有统计学意义(P<0.05);治疗组患者治疗5次、10次后HAMA评分均明显低于同时段的对照组,差异均有统计学意义(P<0.05)。结论电针夹脊穴配合调制中频电治疗老年PHN的疗效、镇痛效果与焦虑缓解效果优于常规局部围刺配合电针治疗,值得临床推广。

Objective To discuss the clinical effect on senile post- herpetic neuralgia( PHN) treated with electroacupuncture( EA) at Jiaji( EX- B2) and modulated medium frequency stimulating therapy in combination. Methods According to random number table,PHN patients were randomized into two groups,38 cases in each one. In the treatment group,EA at Jiaji( EX- B2) and modulated medium frequency stimulating therapy were adopted in combination. In the control group,the routine local surrounding needling and EA were adopted. Separately,before treatment,and in 5,10,15 and 20 times of treatment,VAS and HAMA scores and clinical efficacy were compared between the two groups. Results After two sessions of treatment,the remarkably effective and curative rate in the treatment group was obviously higher than that in the control group,indicating the significant difference( P < 0. 05). VAS scores in the treatment group at each time point were lower than those at the corresponding same time in the control group and than those before treatment.VAS scores after over 10 treatments were lower apparently than those before treatment in the control group,indicating the significant difference( P < 0. 05). In the treatment group,after 5 and 10 treatments,HAMA scores were lower apparently than those in the control group at the same time span,indicating the significant difference( P < 0. 05). Conclusion EA at Jiaji( EX- B2) and modulated medium frequency stimulating therapy achieve the better effect of analgesia and relieving anxiety in senile PHN patients as compared with routine local surrounding needling and EA. This therapy deserves to be promoted in clinical practice.
