葛根芩连汤对2型糖尿病湿热证肠道菌群的影响

The Effect of Gegen Qinlian Decoction on Intestinal Flora in Damp-heat Syndrome of Type 2 Diabetes

目的探讨葛根芩连汤治疗2型糖尿病(T2DM)湿热证患者的临床疗效及其对肠道菌群的影响。方法选取2013年至2014年就诊于上海市浦东医院中医科和内分泌科的110例T2DM湿热证患者为研究对象,按随机数字表法分为中药干预组(50例)和对照组(60例)。对照组患者在饮食控制和运动治疗基础上给予二甲双胍治疗,中药干预组患者在对照组治疗基础上给予葛根芩连汤。观察两组患者空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(Hb A1c)、症状改善及肠道菌群变化情况。结果两组患者治疗后,中药干预组患者的症状改善有效率、FBG、2 h PG、Hb A1c水平及肠道菌群变化情况均明显优于对照组,差异具有统计学意义(P<0.05)。结论葛根芩连汤配合西医常规治疗对T2DM湿热证患者血糖有明显改善作用,其作用机制可能与调节肠道菌群相关。

Objective To investigate the clinical efficacy of Gegen Qinlian Decoction in the treatment of T2 DM patients with damp- heat syndrome and its effect on the intestinal flora. Methods The study objects are the 110 cases of patients in Shanghai Pudong Hospital from 2013 to 2014 who were treated in the traditional Chinese medicine department and endocrine department of damp- heat syndrome in T2 DM. According to the random number table method,the patients are divided into traditional Chinese medicine group( n = 50) and control group( n = 60). Compared to the control group treated with metformin therapy based on the diet control and activity treatment,the treatment group was given Gegen Qinlian decoction treatment additionally. The fasting plasma glucose( FBG),postprandial 2 hours plasma glucose( 2 h PG),glycosylated hemoglobin A1c( Hb A1c),symptom improvement and the improvement of the intestinal flora were observed.Results After treatment,the symptom of the Chinese medicine treatment group was improved with higher efficiency because of the obvious improvement on the FBG,2 h PG,Hb A1 c level and intestinal flora,which is statistically significant( P < 0. 05). Conclusion The therapy of Gegen Qinlian Decoction combined with conventional western medicine has a significant improvement in blood glucose levels with patients having damp- heat syndrome in T2 DM. Its mechanism may be related to the adjustment with the intestinal flora.
