摘要
用微柱凝胶法对8例患者ABO血型进行分析鉴定,通过血清学试验、抗体筛查、吸收放散试验、稀释血清测定反定型对ABO血型正反不符进行综合分析,探讨由于患者产生不规则抗体在用微柱凝胶法鉴定血型时对反定型结果的影响及对临床血型鉴定和安全输血的意义。微柱凝胶法ABO血型正反定型不符,正定型结果为A型3例、B型4例、AB型1例;反定型结果均为O型或待定。吸收放散试验表明,8例患者红细胞上含有A或B抗原,抗体筛查试验和谱细胞红细胞血型抗体鉴定2例产生抗-e抗体,1例产生抗-C抗体,1例产生抗-D抗体,2例产生抗-M抗体,1例产生自身抗体,1例是非特异性凝集。8例患者都产生了不同性质的蛋白质从而影响了微柱凝胶法对ABO血型反定型的鉴定。在临床输血中通过血清学试验、抗体筛查、吸收放散试验,稀释血清测定反定型综合进行血型分析,确保临床输血安全。
8 patients' ABO blood typing was determined by micro column gel technique. Unconformity of positive and reverse on ABO blood typing was analyzed by serological test,antibody screening,absorption elution test and determination of dilution serum reverse typing. The influence of reverse typing results and meaning of clinical serum identification and safe blood transfusion were investigated. ABO blood typing by Micro column gel method did not match. The results of positive typing were that the type A,type B and type AB were seen in 3,4 and 1 cases respectively. The results of reverse typing were type O or undetermined. Absorb radiation test showed that red blood cells contain A or B antigens in 8 patients. Antibody screening and RBC blood group antibody identification suggested that there were anti- e antibody in 2 cases,anti- C antibodies in 1 cases,anti- D antibodies in 1 cases,anti- M antibodies in 2 cases,autoantibodies in 1 case and specific agglutination 1 case. 8 patients had different properties of proteins that had an impact on the micro column gel method of identification of ABO reverse typing. In the clinical blood transfusion practice,blood type need to be analyzed synthetically by serological test,antibody screening,absorb radiation test and diluted serum testing reverse typing to ensure safety of clinical transfusion.
