疑似马钱子中毒2例临床病案报道

Clinical Report of 2 Cases of Suspected Poisoning of Semen Strychni

马钱子具有通络止痛,消肿散结之功,可用于风湿顽痹、跌扑损伤、痈疽肿痛,但其药用剂量与中毒剂量非常接近,民间用肉桂配伍马钱子,有一定减毒增效作用。文中选取2例疑似马钱子中毒临床病案进行分析讨论,报道马钱子中毒的解救方法,并比较分析肉桂与马钱子中毒的异同点。故在临床上使用肉桂解马钱子毒时使用煎剂为宜;其次,临床应个体化用药,并充分考虑个人体质、年龄等的因素,严格控制马钱子剂量,且不可持续久用;第三,其他中药,如马苏、马术,亦能增强马钱子镇痛和抗炎作用,在炮制方面,爆压法、烘焙粉、牛奶热渍法也有一定减毒作用。

Semen strychni acts on promoting circulations,stopping pain and removing mass,indicating intractable rheumatic disease,traumatic injury,carbuncle and furuncle,swelling and pain. The medical dose is very close to the toxic dose. In folk,cinnamon is combined with semen strychi to reduce toxin and increase the effect. In the paper,2 cases of suspected poisoning of semen strychni were discussed,the detoxification method was introduced and the differences between cinnamon poisoning and semen strychni poisoning were compared. In clinical the decoction should be used to detoxify the poisoning. Additionally,the clinical medications should be applied individually with full consideration of body constitution and age. The dose of semen strychni should be controlled strictly and it should not be used persistently. Thirdly,some other herbs,such as sappanwood and atractylodes,may also act on analgesia and anti- inflammation. In herbal processing,the detonation pressure,baking powder and milk staining have a certain detoxification effect.
