逍遥散联合耳针治疗脑卒中后抑郁的疗效评估

Evaluation of therapeutic effects on post-stroke depression treated with xiaoyao san and auricular acupuncture

目的观察逍遥散联合耳针治疗脑卒中后抑郁的临床疗效。方法将68例脑卒中后抑郁肝气郁结证患者随机分为治疗组和对照组各34例,两组均先予基础治疗(内科治疗结合康复训练),然后治疗组予逍遥散加减联合耳针治疗,对照组口服盐酸帕罗西汀片。观察两组患者治疗6周前后汉密尔顿抑郁量表(Hamilton depression scale,HAMD)、卒中量表(China Stroke Scale,CSS)、日常生活活动能力(Activities of daily living,ADL)的评分,评估疗效。结果治疗后,两组患者的总有效率比较,差异有统计学意义(P <0. 05);两组的HAMD、CSS、ADL评分治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P <0. 05)。治疗组没有发生不良反应。结论在基础治疗后采用逍遥散联合耳针治疗脑卒中后抑郁能有效、安全改善肝气郁结证患者的临床症状,提高患者的ADL,临床疗效优于口服西药治疗。

Objective To observe the clinical effects on post-stroke depression treated with xiaoyao san and auricular acupuncture.Methods A total of 68 patients of post-stroke depression of liver qi stagnation were randomized into a treatment group and a control group,34 cases in each one.The basic treatment(internal medicine treatment and rehabilitation training)was provided in the two groups.Additionally,in the treatment group,the modified xiaoyao san and auricular acupuncture was used and in control group,paroxetine hydrochloride tablets were taken orally.Before and after treatment,the scores in Hamilton depression scale(HAMD),China stroke scale(CSS)and activities of daily living(ADL)were observed after 6-week treatment and before treatment in the two groups.The therapeutic effects were evaluated.Results After treatment,the total effective rates were different significantly between the two groups(P<0.05).The scores of HAMD,CSS and ADL were different significantly before and after treatment in the self-comparison of each group as well as between the groups after treatment(P<0.05).No adverse reaction happens in the treatment group.Conclusion Besides the basic treatment,the combined therapy of oral administration of xiaoyao san and auricular acupuncture effectively and safely relieves the clinical symptoms of liver qi stagnation and improves ADL in the patients with post-stroke depression.The clinical effects are better than oral administration of western medicine.
