化纤胶囊治疗脓毒症急性肺损伤的临床观察

Clinical observation of acute lung injury with sepsis treated wih Huaxian capsule

目的通过对脓毒症急性肺损伤患者在西医常规治疗基础上联合化纤胶囊治疗后的相关临床指标分析,探讨化纤胶囊对脓毒症急性肺损伤患者的临床疗效。方法分析黑龙江中医药大学附属第一医院重症医学科2016年10月至2018年5月确诊的60例脓毒症急性肺损伤气虚血瘀型患者临床病例资料,采用随机数字表法将上述60例患者随机分为对照组和治疗组两组,将单纯行西医常规基础性治疗的30例脓毒症急性肺损伤患者设为对照组,将在西医常规治疗基础上联合化纤胶囊治疗的30例脓毒症急性肺损伤患者设为治疗组,评价并比较两组患者经治疗前后的APACHEⅡ评分、Murray肺损伤评分、体温、动脉血氧分压的变化以及中医证候积分改善情况。结果治疗后,两组患者的APACHEⅡ评分、Murray肺损伤评分及体温均比治疗前显著降低(P <0. 05),且治疗组降低程度显著高于对照组(P <0. 05);两组患者的动脉血氧分压显著升高(P <0. 05),且治疗组升高程度显著高于对照组(P <0. 05);两组患者治疗前后中医证候疗效比较,治疗组疗效优于对照组,差异有统计学意义(P <0. 05)。结论在西医常规治疗基础上联合化纤胶囊治疗脓毒症急性肺损伤,对脓毒症急性肺损伤的疗效优于对照组,临床上选用中药化纤胶囊,能有效改善脓毒症急性肺损伤患者的生命状态,为临床治疗脓毒症急性肺损伤提供了新的思路和方法。

Objective To study the effect of acute lung injury with sepsis treated with Huaxian capsule and western medicine though analyzing its clinical indicators.Methods From October 2016 to May 2018,60 sepsis patients with acute lung injury with qi deficiency and blood stasis type were divided into control group and treatment group by random number table method.30 sepsis patients with acute lung injury treated with conventional basic treatment of Western medicine alone were divided into control group and 30 sepsis patients with acute lung injury treated with Huaxian capsule on the basis of conventional treatment of Western medicine were divided into treatment group.The changes of APACHE II score,Murray lung injury score,body temperature,arterial oxygen partial pressure and TCM syndrome points before and after treatment were evaluated and compared.Results After treatment,the APACHE II score,Murray lung injury score and body temperature were significantly lower than those before treatment(P<0.05),and the reduction in the observation group was significantly higher than that in the control group(P<0.05).The arterial oxygen partial pressure was significantly increased(P<0.05),and the elevation of the observation group was significantly higher than that of the control group(P<0.05),indicating the significant difference(P<0.05).The efficacy of TCM syndromes in the treatment group was better than that in the control group,indicating the significant difference(P<0.05).Conclusion Huaxian Capsule can effectively improve the life state of sepsis patients with acute lung injury,and provide new thoughts and methods for clinical treatment of this disease.
